Öğe 17p Deletion is associated with resistance of B-cell chronic lymphocytic leukemia cells to in vitro fludarabine-induced apoptosis(Taylor & Francis Ltd, 2007) Turgut, Burhan; Vural, Ozden; Pala, Funda S.; Pamuk, Gulsum E.; Tabakcioglu, Kiymet; Demir, Muzaffer; Ongoren, SenizWe explored the relationship between the cytogenetic/biologic characteristics of B-chronic lymphocytic leukemia (B-CLL) cells and their tendency to undergo spontaneous or fludarabine-induced apoptosis in vitro. B cells from 36 B-CLL patients were incubated with or without fluclarabine for 48 h. Apoptosis was determined by two assays: annexin V staining and DNA staining. Fluorescence in situ hybridization was used for detection of trisomy 12, 11q deletion, and 17p deletion. Bcl-2 and CD38 expressions were determined by flow cytometry. Five patients had 17p deletion, 6 had trisomy 12, and another 6 had 11q deletion. B-CLL cells with 17p deletion had significant resistance to apoptosis induced by fludarabine and a slight spontaneous resistance to apoptosis. Bcl-2 and CD38 were not associated with in vitro spontaneous and fludarabine-induced apoptosis. In conclusion, 17p deletion, which causes loss of p53 gene, is associated with resistance to fludarabine-induced apoptosis in vitro. New treatment modalities should be tried in B-CLL patients with 17p deletion.Öğe Acute Lymphoblastic Leukemia; Prognostic Factors' Perspective(Akad Doktorlar Yayinevi, 2010) Tekgunduz, Emre; Demir, Muzaffer; Akpinar, SevalAcute lymphoblastic leukemia represents a biologically and clinically heterogeneous group of diseases with different outcomes among patients subgroups. Identification of prognostic factors is critical in selecting therapy for subgroups of patients to achieve a high cure rate with minimal toxicity. Another important goal of risk stratification is to define patients with a very high risk of relaps, who may benefit from up-front allogeneic hematopoietic stem cell transplantation following first complete remission. Clinical parameters, immunophenotype, cytogenetics, treatment response, minimal residual disease are among currently used factors in risk stratification of acute lymphoblastic leukemia patients. But there are emerging new parameters like global gene expression profiling, host pharmacodynamics-pharmacogenetics and treatment protocols which may have an impact on risk-adapted treatment in the near future. In the present review we will discuss a variety of prognostic factors and their impact on clinical outcome of children and adults with acute lymphoblastic leukemia.Öğe The alteration of NTproCNP plasma levels following anaerobic exercise in physically active young men(Turkish Soc Cardiology, 2015) Temur, Hilal Akseki; Vardar, Selma Arzu; Demir, Muzaffer; Palabiyik, Orkide; Karaca, Aziz; Guksu, Zuhal; Ortanca, ArifObjective: Amino-terminal propeptide of C-type natriuretic peptide (NTproCNP) is a synthesis product of C-type natriuretic peptide (CNP). In this study, plasma levels of NTproCNP were compared before and after exercise in healthy young subjects who are physically active (PA) or not physically active (NPA). Methods: The study was carried on PA group (n=10) who defined the exercise duration more than 2.5 hours per week for at least one year and NPA group (n=10) whose exercise duration was lower than 1.5 hours per week. The level of maximal oxygen consumption was determined. Wingate exercise test was applied on the following day. Plasma NTproCNP levels were measured before the exercise and at the 1st, 5th and 30th minute after the exercise. Results: Exercise duration of physically active group was reported as 11.3 +/- 5.0 hours per week. Basal NTproCNP levels of the groups were found to be comparable. NTproCNP levels in the 5th minute (0.93 +/- 0.23 pmol/L; p<0.05) and in the 30th minute (0.77 +/- 0.21 pmol/L p<0.05) after exercise were higher than the levels before exercise (0.64 +/- 0.29 pmol/L) in PA group. Additionally, the plasma levels of NTproCNP after 5th minute of exercise were higher in PA group (0.93 +/- 0.23 pmol/L) than NPA group (0.74 +/- 0.16 pmol/L, p<0.05). Conclusion: Being physically active may be a fact affecting the secretion of CNP, which plays a protective role in endothelium, following exercise.Öğe Antithrombotic and Anticoagulant Usage Policies(Aves Yayincilik, Ibrahim Kara, 2010) Demir, Muzaffer; Tekguenduez, EmreHeparin, heparin-derived compounds and warfarin have been successfully used for the prevention and treatment of venous thromboembolism during last 50 years. Low-molecular-weight heparins are alternative drugs to unfractionated heparin because of pharmacokinetic and pharmacodynamic features nowadays. Warfarin is still a gold standard anticoagulant oral agent. However, there are some limitations of these mentioned drugs. It is needed the new oral and parenteral anticoagulant drugs to solve these limitations. There are several preclinical and clinical studies on efficacy and safety of new drugs such as parenteral FXa inhibitors (idraparinux and idraparinux-biotin complex) and oral FXa inhibitor (rivaroxaban-Xiralto) and oral Flla inhibitor (dabigatran-Pradaxa). Both rivaroxaban and clabigatran are newly approved for short term usage on the prevention of VTE during orthopedic procedures such as hip and knee replacement.Öğe Aspergillus spp. isolations from respiratory tract samples in Trakya University Hospital(Turkish Assoc Tuberculosis & Thorax, 2007) Gurcan, Saban; Demir, Muzaffer; Altiay, Gundeniz; Tikvesli, Melek; Kilic, Haluk; Otkun, MetinThe characteristics of cases diagnosed as aspergillosis and Aspergillus spp. strains isolated from the respiratory tract samples in Mycology Laboratory of Trakya University Hospital between January 2002 and May 2006 were investigated. In this period, 137 bronchoalveolar lavages, 95 sputum, nine tracheal aspirates, three lung biopsies and one bronchial biopsy of 85 patients were processed. The samples were incubated in 25 degrees C and 35 degrees C media by culturing on brain heart infusion agar with blood and Sabouraud dextrose agar. Presence of leucocytes and fungal structures were searched in the smear stained by Gram and Giemsa. The patient was defined as probable aspergillosis case, if he/she patient had clinical findings, lung infiltration or fungus ball radiologically, at least one risk factor predisposing to aspergillosis and isolation of Aspergillus spp. in lower respiratory tract samples without finding of other nonmycotic infection. Of 22 patients isolated Aspergillus spp., 13, six, two, one were internalized in chest diseases, haemotology, neurosurgery and oncology clinics, respectively. Seven positive cultures were considered as findings of aspergillosis. Aspergillus fumigatus, Aspergillus flavus and Aspergillus niger were isolated in three, two, and two patients, respectively. Fungal structures were detected in only one sample in the direct microscopical examination. Ages of seven patients, five were males and two were females, were between 15 and 60. Predisposing risk factors were acute leukemia in six patients and lung cancer in one patient. Five patients were neutropenic and one was neutrophylic. Fungus ball was detected in radiological imaging of one patient, had a pulmonary cavitary lesion. Conventional amphotericine B was used in their therapies. Antifungal agents were switched to caspofungin and itraconazole in two and one patients, respectively. Three patients died in four weeks after isolation of Aspergillus spp. Aspergillosis cases were not high in our hospital because of absence of transplantation center for bone marrow or solid organ.Öğe A biomarker and therapeutical target in immune thrombocytopenia: TNF receptor-associated factor 6(Future Medicine Ltd, 2019) Asoglu, Veysi; Umit, Elif G.; Demir, MuzafferBackground: Immune thrombocytopenia (ITP) is an acquired disease characterized by various pathogenetic mechanisms. We aimed to evaluate the value of TRAF6 as a biomarker of pathogenesis in ITP. Methods:85 newly diagnosed ITP patients and 85 controls were included. Results: In the patient group, mean TRAF6 level was 2348pg/ml and in the control group, TRAF6 level was 25.57pg/ml. In corticosteroid-responding patients, TRAF6 levels were lower than nonresponding patients. Discussion: TRAF6 levels were observed to be higher than healthy controls, which suggest a solid relation with ITP pathogenesis. In addition, we describe the spectrum of other skin lesions, including, secondary skin malignancies and nonspecific benign skin lesions in haematological malignancy patients. MethodsWe defined 58 skin lesions that developed in 54 inpatients hospitalised in the Department of Haematology, Trakya University Medical Faculty, Turkey. All skin lesions that developed in inpatients between 2006 and 2012 had been evaluated by a dermatologist. The patients' clinical features, skin biopsy results and therapies were obtained from hospital files. The diagnosis of LC was based on clinical features and histopathological examinations of the skin biopsy. ResultsThere were 11 patients with LC. Six (54.5%) had acute myeloblastic leukaemia. In nine patients (82%), LC was present at the initial presentation. Secondary skin malignancy was detected in 11 patients (five basal cell carcinoma, four Kaposi's sarcoma, one squamous cell carcinoma, one malignant melanoma); and malignancy was present in two patients (18%) at the initial presentation. Nonspecific benign skin lesions, the most frequent of which were drug eruptions, were determined in 32 of our patients. LC had a significantly higher likelihood of being present at initial presentation than other skin lesions (P<0.01). The median survival in LC patients was quite short (4.5 months). ConclusionsLC was usually diagnosed at the initial presentation of the patient or during the early course of the disease. Cognitive problems can appear in advanced stage of COPD. There is relationship between COPD and levels of homocystein (hcy) and total antioxidant capacity (TAC). There is also relationship between cognitive dysfunction and levels of hcy and TAC. We aimed evaluation of cognitive functions in patients with COPD, and demonstration of the relationship between cognitive function and hcy and TAC. Methods: Twenty-eight hypoxic (group 1), 28 non-hypoxic COPD patients (group 2) and 25 healthy people (control group, group 3) were included to study. Hcy and TAC levels were measured in all subjects. Cognitive functions were evaluated in all subjects with standardized mini mental test (SMMT), clock drawing test (CDT), Blessed orientation memory concentration test (BOMCT), Benton's facial recognition test (BFRT) and clinical dementia rating scale (CDRS). Results: Hcy levels were high in group 1 (p=0.004), TAC levels were high in group 1 and 2 (p=0.04). SMMT (p=0.000) and CDT values were low in group1 (p=0.000), CDRS values were high in group1 and 2 (p=0.000), BFRT values were low in group1 and 2 (p=0.000), BOMCT values were high in group1 (p=0.000). There were a correlation between TAC and SMMT (r=0.582, p=0.001) and CDRS (r=-0.384, p=0.044). Conclusions: We demonstrated presence of cognitive dysfunction in COPD patients. Patients with hypoxic had more profound cognitive dysfunction. In addition, we demonstrated a relationship between TAC and cognitive dysfunction.Öğe Correlation Between Maternal Plasma Thrombomodulin and Infant Birth Weight in Hypertensive Disorders of Pregnancy(Sage Publications Inc, 2009) Varol, Fuesun G.; Ozgen, Levent; Sayin, N. Cenk; Demir, MuzafferObjective: To evaluate the association between maternal plasma thrombomodulin levels and infant birth weights in pregnancy-induced hypertension. Study design: Plasma thrombomodulin levels were measured in 80 pregnat women living in the Trakya region of Turkey. Kruskal-Wallis statistical analysis was performed in comparison of the descriptive and laboratory data (significance at P<.05). Results: The plasma thrombomodulin values in hypertensive disorders in pregnancy were found to be highly correlated with the infant birth weights(P<.001). Conclusion: Elevated plasma thrombomodulin levels in hypertensive disorders of pregnancy were well correlated withh related infant birth weights of these pathhologies. The aim of the present study is to investigate the predictive value of fibronectin in determining the presence and severity of CAD. Patients with stable angina (n = 62) who had angiographically documented CAD, and control patients (n = 31) who had normal coronary angiograms, were included in the study. Plasma fibronectin levels were determined in all patients. Plasma fibronectin level (milligrams per liter) in patients with CAD was higher than normal controls (364.2 +/- 171 vs 265.1 +/- 135.5, p = 0.006). The severity of CAD determined according to Gensini score and fibronectin level did not show any correlation (r = 0.13, p = 0.311). If fibronectin level 240 mg/l was determined as cutoff, it showed 76% sensitivity, 46% specificity, 46% negative predictive value, and 72.3% positive predictive value for predicting CAD. The present study showed that plasma fibronectin level in CAD is significantly higher than normal control subjects. However, it has no role in predicting the severity of CAD.Öğe Differences in platelet-leukocyte aggregates among subtypes of acute cerebral ischemia(Elsevier, 2011) Turgut, Burhan; Turgut, Nilda; Celik, Yahya; Tekgunduz, Emre; Pamuk, Gulsum Emel; Demir, MuzafferBackground: Acute cerebral ischemia is caused by different pathophysiological mechanisms. The role of platelets and other blood cells can be different among the stroke subtypes. Methods: Seventy-two patients with acute ischemic cerebrovascular disease, including 31 patients with large vessel disease, 21 patients with cardioembolic disease, and 20 patients with small vessel disease, were evaluated. P-selectin (CD62P) expression and platelet leukocyte aggregates were measured with flow cytometry at the acute phase after the ischemic event. Markers were also measured in 37 control subjects. In all subjects, the serum high-sensitivity C-reactive protein (CRP) was also measured. Results: The platelet-monocyte aggregates (PMA) and platelet-granulocyte aggregates (PGA) in the large vessel disease group were higher than in control group (P=0.002, and P < 0.0001, respectively). The PMA and PGA in the small vessel disease group were also higher than in the control group (P=0.004 and P < 0.0001, respectively). In contrast, in the cardioembolic disease group, the PMA and PGA were not significantly different from the control group. CD62P expression was higher in all of the patient groups relative to the control group (P < 0.05 for all comparisons). Serum CRP levels were also higher in all of the patient groups than in the control group (P < 0.0001 for all comparisons). Conclusions: In contrast to large vessel and small vessel disease, it seems that platelet-leukocyte association does not play a crucial role in the pathogenesis of cardioembolic stroke. In this study, we evaluated platelet and endothelial activation parameters in AS patients. Fiftynine AS patients and 22 healthy controls were included. The clinical features and acute phase parameters were evaluated. In all patients and healthy controls, platelet-monocyte complexes (PMC), platelet-neutrophil complexes, basal and ADP-stimulated P-selectin (CD62P) expression were determined by flow cytometry; soluble E-selectin (sE-selectin) and soluble CD40L (sCD40L) were determined by ELISA. AS patients were divided into two groups as active and inactive by using BASDAI. In 15 AS patients, the evaluated parameters were assessed before and after 12 weeks of anti-TNF therapy. PMC and sCD40L levels in AS patients were significantly higher than in the control group (P values 0.013 and 0.016). The evaluated variables were similar in active and inactive AS groups (P > 0.05). There were no significant changes in platelet and endothelial activation parameters in AS patients after anti-TNF therapy (P > 0.05). Platelet activation which is reflected by high levels of PMC and sCD40L might be responsible for the increased frequency of atherosclerosis in AS. The platelet activation in our AS patients was not associated with disease activity and did not improve after anti-TNF therapy.Öğe Dorsal sural nerve conduction study in vitamin B12 deficiency with megaloblastic anemia(Blackwell Publishing, 2006) Turgut, Burhan; Turgut, Nilda; Akpinar, Seval; Balci, Kemal; Pamuk, Guelsuem E.; Tekguenduez, Emre; Demir, MuzafferPeripheral neuropathy is frequently observed in B-12 deficiency. In spite of this, there is little knowledge about peripheral neuropathy in B-12 deficiency because the severity of clinical involvement of the central nervous system clearly outweighs signs and symptoms due to peripheral nervous system involvement. We primarily investigated peripheral neuropathy with dorsal sural conduction study, which is a new method for detection of early peripheral neuropathy, in B-12 deficiency with megaloblastic anemia. Conventional nerve conduction studies and tibial sensory-evoked potential (SEP) recording were also performed. Twenty-eight B-12-deficient patients (15 male, 13 female, mean age 65.8 years) with megaloblastic anemia and 18 age- and sex-matched controls were included in the study. Although dorsal sural sensory nerve action potentials (SNAPs) were not recorded in 15 (54%) of 28 patients, only 9 (32%) of them were found to have polyneuropathy by conventional conduction studies. Furthermore, patients with dorsal sural SNAP had mean lower amplitude, mean longer latency, and slower velocity response when compared with controls. Twenty patients (71%) were diagnosed as having myelopathy by the combination of tibial SEP and neurological findings. Two patients whose dorsal sural SNAPs were not recorded had normal tibial SEP responses; therefore, these patients were considered to have isolated peripheral neuropathy. As a result, we conclude that dorsal sural nerve conduction study is a reliable method for detection of early peripheral neuropathy in B-12 deficiency.Öğe The effects of nebivolol on fibrinolytic parameters in mild and moderate hypertensive patients(Pulsus Group Inc, 2007) Tarighi, Bahman; Kurum, Turhan; Demir, Muzaffer; Azcan, Sen NurBACKGROUND: The aim of the present study was to investigate the effects of nebivolol (5 mg daily) on plasma levels of hemostatic and fibrinolytic endothelial function markers in mild or moderate hypertensive patients. METHODS AND RESULTS: Thirty-five (22 female, 13 male; mean +/- SD 54.7 +/- 11.3 years of age) mild and moderate hypertensive patients were included the study. The mean systolic blood pressure [BP] was 160 mmHg (range 150 mmHg to 165 mmHg) and the mean diastolic BP was 100 mmHg (range 90 mmHg to 100 mmHg). Plasma tissue plasminogen activator antigen (tPA-Ag), plasminogen activator inhibitor type 1 antigen (PAI-1-Ag), PAI-1-activity, tPA-Ag/PAI-1-Ag index, fibrinogen and euglobulin lysis time were determined before and after two months of therapy. tPA-Ag and PAI-Ag levels were measured by ELISA. After this period, treatment with nebivolol (5 mg/day) in all patients was associated with a significant decrease in systolic BP and diastolic BP (P < 0.001 for each), heart rate (P < 0.01), fibrinogen (P < 0.005) and euglobutin lysis time (P < 0.01). The tPA-Ag and tPA-Ag/PAI-1-Ag index levels were increased significantly (P < 0.001 for each) in all patients, but the PAI-1-Ag (P > 0.05) and PAI-1 activity (P > 0.05) did not show significant change. In the present study, there was no correlation between decreases in arterial BP and decreases in fibrinolytic parameters (P > 0.05), but there was a positive, statistically significant correlation between fibrinogen and body mass index (P < 0.001). Twenty patients with PsA, eight psoriasis and 20 healthy controls were included into the study. The patients' clinical features and acute phase parameters were assessed. In all patients and controls, platelet-monocyte complexes (PMC), platelet-neutrophil complexes (PNC), and basal and ADP-stimulated P-selectin expression were determined with flow cytometry; soluble E-selectin (sE-selectin) and soluble CD40L (sCD40L) were determined with ELISA. Patterns of joint involvement and degrees of skin involvement in PsA patients were assessed. PMC in PsA patients were significantly higher than in the control group (p = 0.02). PNC were not significantly different among the three groups (p values > 0.05). sE-selectin levels in both PsA and psoriasis groups were significantly higher than in healthy controls (p values, respectively, <0.001 and 0.023). Basal and ADP-stimulated CD62P expression and sCD40L level were similar in all groups (p values > 0.05). Polyarticular PsA patients had significantly higher sCD40L than oligoarticular plus spondylitic PsA groups (p = 0.04). sCD40L level was higher in active PsA group than in inactive PsA group (p = 0.03). Groups with limited and extensive skin involvement did not differ significantly in the evaluated parameters. C-reactive protein (CRP) level in PsA patients correlated with sCD40L (r = 0.69, p = 0.012), basal CD62P expression (r = 0.89, p < 0.001) and ADP-stimulated CD62P expression (r = 0.73, p = 0.001). Endothelial activation might be have a role in the pathogenesis of both psoriasis and PsA. One hundred and forty patients with a diagnosis of a hematological malignancy completed the Hospital Anxiety and Depression Scale (HADS) and the General Health Questionnaire. Patients with higher anxiety scores were more frequently inpatients, had higher EORTC general symptom scores, and they had lower cognitive, emotional, social functioning and global quality of life (QoL) scores (all p values < 0.05). Patients with higher depression scores had more frequently active disease and were inpatients; they had higher mean Eastern Cooperative Oncology Group performance scores, EORTC gastrointestinal system and general symptom scores, and significantly lower physical, role, emotional, social and cognitive functioning and global QoL scores (all p values < 0.01). During follow-up, it was observed that survival curves of patients with active disease who had higher HADS depression scores tended to be shorter than those with lower scores (p=0.1). Anxiety and depression are frequent in hematological malignancy patients and associated with poor QoL and performance status. In addition, the presence of self-reported depression might have a predictive value for poor prognosis.
