Carcinoma arising from a thyroglossal duct cyst is a rare entity, the most common histological type being papillary carcinoma. A 23-year-old male patient presented with a painless mass at the level of the hyoid bone in the midline of the neck. It was mobile on swallowing. With an initial diagnosis of thyroglossal duct cyst, the lesion was removed with the Sistrunk procedure. Histopathological examination of the surgical specimen showed papillary carcinoma arising from the thyroglossal duct cyst. Thyroid suppression therapy was initiated postoperatively.
