Özet
Sağlık hizmetlerinin kaliteli olabilmesi için ilkin sağlık çalışanlarının ruhsal ve fiziken sağlıklı olması, işini sevmesi, stresle baş edebilmesi ve güvende hissetmesi gerekir. Buna istinaden, bu çalışma Edirne il merkezindeki aile hekimlerinin, işe bağlı gerginlik düzeylerini ve hekimlerinin tükenmişliklerini belirlemek, bunları etkileyebilecek bazı sosyodemografik veriler ile bağlantısını bulmak amacıyla yapılmıştır. Bu çalışma ile birlikte aile sağlığı merkezlerinde çalışan aile hekimlerinin toplumdaki konumunun iyileştirilebilmesi, hekimlere işe bağlı gerginlik ve tükenmişlikle baş etme konusunda eğitim verilebilmesi, medyanın aile hekimleri üzerindeki olumsuz etkisi azaltılabilmesi, aile sağlığı merkezlerinin çalışma koşullarının düzeltilmesine dair yapılacak çalışmalar için önemli bilgiler sunmak hedeflenmiştir. Çalışmamıza katılan 52 aile hekiminin 25’i (%48) erkek, 27’si (%52) kadınlardan oluşmaktadır. 52 aile hekiminin yaş ortalaması 47,36 ± 7,02 iken, ortalama hizmet yılı süresi 22,3 ± 7,10 olarak saptanmıştır. Poliklinikte günlük bakılan ortalama hasta sayıları sorulduğunda 30 ile 60 arasında hasta sayısı bakan aile hekimleri %61 (n=32) oranında iken, 60 hastanın üzerindeki cevap sayısı %37 (n=19) ve %1 (n=1) oranında ise 30 hastanın altında cevabı alınmıştır. Çalışma ortamındaki şiddet sorusuna verilen yanıt ise fiziksel şiddet için %19 oranında (n=10) evet, %81 (n=42) oranında hayır yanıtı alınırken; sözel şiddette bu oran tam tersi olarak karşımıza çıkmış %81 (n=42) evet, %19 (n=10) oranında ise hayır olarak saptanmıştır. Gelecekle ilgili mesleksel kaygıları sorulan aile hekimleri %91 (n=47) oranında kaygıları olduğunu, %9 (n=5) oranında kaygıları olmadığını belirtmiştir. Sonrasında sorulan çalışma ortamından memnuniyet sorusuna aile hekimlerinin %67 oranında (n=35) memnun olduklarını belirtmiş, %33 (n=17) memnun olmadıklarını belirtmiştir. İş beklentinizi karşılıyor mu, sorusuna verilen yanıtlar %34 (n=18) oranında evet, %66 (n=34) oranında hayır olarak saptanmıştır. En son olarak fiziksel ortamın yeterliliği ve beklentilerini karşılayıp karşılamadığı sorulduğunda aile hekimlerinin verdikleri cevap %52 (n=27) oranında beklentilerini karşıladığı yönündeyken, %48 (n=25) oranında beklentilerini karşılamadığına dair olumsuz yanıt vermişlerdir. Tükenmişlik ve gerginlik ölçeklerine bakıldığında aile hekimlerinin %76,9’u orta seviyede işe bağlı gerginlik yaşamaktadır. Maslach alt ölçeklerinde ise aile hekimlerinin %75,0’ı düşük kişisel başarı yaşarken, %94,2’si yüksek seviyede duygusal tükenme yaşamaktadır. İstatistiki analizlerde ise duygusal tükenme ise meslekte hizmet yılı arasında anlamlı bir pozitif korelasyon bulunmuştur. Çalışmamızla birlikte elde edilen bulgular değerlendirildiğinde, aile hekimlerinin ilkeleri ve prensipleri de dikkate alındığında memnuniyetleri olsa bile, kaygılarının yüksek olduğu açıktır. Orta derecede işe bağlı gerginlik, yüksek seviyede yaşanan duygusal tükenme ve düşük kişisel başarının aile hekimleri arasındaki yaygınlığı dikkate alınması gereken bir konudur. Ayrıca da aile hekimlerin iş yüklerinin fazlalığı göz önüne alınırsa tükenmişlik açısından daha sıkı önlemler alınmalı ve bu gibi çalışmalar daha büyük ölçekli yapılarak, çalışma koşulları iyileştirilmelidir.
In order for health services to be of high quality in today's conditions, first of all, health workers must be mentally and physically healthy, be protected from burnout, be able to do their job with pleasure, cope with stressful situations effectively and feel safe. Based on this, in this study, it was aimed to determine the burnout and work-related stress levels of family physicians working in family health centers in Edirne centrum and to investigate the relationship with some demographic characteristics that may affect them. With this study, it is also aimed to provide important information for the studies to be done to improve the family physicians working conditions in family health centers, to train physicians to cope with work-related stress and burnout, to reduce the negative impact of the media on family physicians, and to improve the working conditions of family health centers. Of the 52 family physicians participating in our study, 25 (48%) were male and 27 (52%) were female. While the mean age of 52 family physicians was 47.36 ± 7.02, the average length of service was found to be 22.3 ± 7.10 years. When the average number of patients seen daily in the outpatient clinic was questioned, the rate of family physicians who cared for the number of patients between 30 and 60 was 61% while over 60 patients was 37% and 30 patients were %1. The answer given to the question of violence in the working environment was 19% for yes; and 81% for no. However, in verbal violence, this rate was found to be the opposite, 81% for yes and 19% for no. %91 of family physicians, had professional concerns about their future. When asked about the satisfaction with the working environment, 67% of the family physicians stated that they were satisfied, and 33% stated that they were not satisfied. The answers given to the question "Does the job meet your expectations?" were found to be yes at a rate of 34% and no at a rate of 66%. Finally, when asked whether the physical environment meets the adequacy and expectations, the answer given by the family physicians was that it met their expectations at a rate of 52%, while 48% gave a negative answer. Considering the burnout and tension scales, 76.9% of family physicians experience moderate work-related stress. On the Maslach Burnout Inventory subscales, 75.0% of family physicians experience low personal accomplishment, while 94.2% experience high levels of emotional exhaustion. In statistical analysis, a significant positive correlation was found between emotional exhaustion and years of service in the profession. When the findings obtained with our study are evaluated, it is clear that family physicians have high concerns, even if they are satisfied when their principles and principles are taken into account. The prevalence of high levels of emotional exhaustion and low personal achievement among family physicians is an issue that needs to be taken into account. In addition, considering the excessive workload of family physicians, stricter measures should be taken in terms of burnout and such studies should be carried out on a larger scale and working conditions should be improved.
