Abstract
Bu çalışmamızda amacımız, poliklinik takipli hastalarımızın çoğunu oluşturan "Dikkat Eksikliği ve Hiperaktivite Bozukluğu" tanılı olgularda en sık kullandığımız farmakolojik tedavi ajanı olan metilfenidatın büyüme ve kemik yapısı üzerine etkilerini araştırmak olmuştur. Trakya Üniversitesi Tıp Fakültesi, Çocuk ve Ergen Ruh Sağlığı ve Hastalıkları Anabilim Dalı tarafından Dikkat Eksikliği ve Hiperaktivite Bozukluğu tanısı ile takip ve tedavi edilen; 2 (iki) yıldan uzun süre metilfenidat kullanım öyküsü mevcut olan, 8 (sekiz) -16 (onaltı) yaş grubu 30 erkek vaka, olgu grubu olarak çalışmaya dahil edilmiştir. Edirne Devlet Hastanesi Ortopedi ve Travmatoloji polikliniğine başvurmuş, psikiyatrik tanı ve tedavi öyküsü bulunmayan, anti-epilepik ilaç kullanım öyküsü bulunmayan, uzun süreli (>6 ay) ilaç kullanım öyküsü bulunmayan 8 (sekiz) -16 (onaltı) yaş grubu 30 erkek vaka, kontrol grubu olarak çalışmaya dahil edilmiştir. Gruplar; yaş (yıl), boy uzunluğu (cm), vücut ağırlığı (kg) ve vücut kitle indeksi (kg/m2 ), 25-hidroksikolekalsiferol (Vitamin D), serum kalsiyum değerleri (mg/dl) ve DEXA (Dual enerjili x-ray absorpsiyometrisi) sonuçları ile karşılaştırılarak değerlendirilmiştir. Serum kalsiyum düzeyleri, proksimal femur ve lomber vertebra Z skorlarının olgu grubunda istatistiksel olarak anlamlı düzeyde düşük olduğu tespit edilmiştir (p<0,001; p=0,013; p=0,015). In this study, we aimed to investigate the effect of methylphenidate on growth and bone structure at patients of Attention Deficit Hyperactivity Disorder who are the vast majority of our outpatients. Thirty male patients who medicated with methylphenidate at least 2 years, between 8- 16 years old, diagnosed and treated for Attention Deficit Hyperactivity Disorder at Child and Adolescent Psychiatry Department of Medical Faculty Hospital of Trakya University were included to study as case group. Thirty male patients who had no psychiatric diagnosis or treatment, no medical history for antiepileptic drugs, no medical history for long-term (>6 months) medication, between 8-16 years old, examined in outpatient clinic of Orthopedics and Traumatology in Turkish Minister of Health, Edirne Sultan 1.Murat State Hospital were included to study as control group. Groups were compared and evaluated with age (years), height (cm), body weight (kg), body mass index (kg/m2 ), 25-hydroxycholecalciferol (Vitamin D), serum calcium levels (mg/dl) and DEXA (Dual Energy X-ray Absorptiometry) results. Serum calcium levels, proximal femur and lumbar vertebra Z scores were significantly lower in the case group. (p<0,001; p=0,013; p=0,015). Keywords: ADHD, methylphenidate, DEXA, bone, growth.
