Özet
Çalışmada, cerrahi operasyon geçirecek hastaların ruhsal iyilik durumunu etkileyen psikososyal etkenleri tanımlamak amaçlanmıştır. Kesitsel tipteki bu araştırma 15.01.2021-15.07.2021 tarihleri arasında Trakya bölgesinde bulunan bir devlet hastanesinin ortopedi, genel cerrahi, beyin cerrahisi ve kadın doğum servislerinde ameliyat olmayı bekleyen 236 hastayla gerçekleştirildi. Araştırma verileri anket formu, WHO-5 İyilik Durumu İndeksi ve Algılanan Stres Ölçeği ile toplandı. Verilerin değerlendirilmesinde sayı, yüzde, ortalama, standart sapma, bağımsız gruplarda t testi, tek yönlü varyans analizi, tukey çoklu karşılaştırma testi ve doğrusal regresyon analizi kullanıldı. Cerrahi operasyon geçirecek hastaların WHO-5 İyilik Durumu İndeksi toplam puan ortalaması ve standart sapması 10,76±6,21 olarak bulundu. Çalışmaya katılan hastaların WHO5 İyilik Durumu İndeksi puanları ile Algılanan Stres Ölçeği puanları arasında istatistiksel olarak anlamlı, negatif yönlü ilişki saptanırken, hastaların WHO-5 İyilik Durumu İndeksi toplam puanlarının ekonomik durum, kronik hastalık durumu, ameliyat öyküsü, daha önce psikiyatrik ilaç veya psikolojik destek alma durumu, sigara kullanımı, son 3 ayda yaşamında önemli bir değişiklik olma durumu ve günlük yaşamında sık sık stres yaşama durumu değişkenlerine göre farklılık gösterdiği belirlendi (p<0,05). Sonuç olarak cerrahi operasyon geçirecek hastaların ruhsal iyilik durumunun düşük düzeyde olduğu, hastaların tanıtıcı özellikleri ile algıladıkları stres düzeylerinin ruhsal iyilik durumu üzerinde etkileri olduğu saptandı. Bu sonuçlar doğrultusunda, preoperatif dönemde hastalar ruhsal iyilik durumunu etkileyen psikososyal etkenler konusunda hemşirelerin farkındalığının artırılması ve ameliyat öncesi psikososyal değerlendirmenin rutin hemşirelik uygulamalarına dahil edilmesi önerilmektedir.
In this study, it was aimed to determine the psychosocial factors that affect the mental well-being of patients who will undergo surgical operation. This cross-sectional study was conducted with 236 patients waiting for surgery in the orthopedics, general surgery, neurosurgery and gynecology services of a state hospital in the Thrace region between 15.01.2021 and 15.07.2021. Research data were collected with a questionnaire, WHO-5 Well-Being Index and Perceived Stress Scale. Number, percentage, mean, standard deviation, independent groups t-test, one-way analysis of variance, Tukey multiple comparison test and linear regression analysis were used to evaluate the data. The WHO-5 Well-Being Index total score and standard deviation of the patients who will undergo surgery were found to be 10.76±6.21. While a statistically significant and negative correlation was found between the WHO-5 Well-Being Index scores and the Perceived Stress Scale scores of the patients participating in the study, were related to their economic status, chronic disease status, history of surgery, previous psychiatric medication or have received psychological support, smoking, having a significant change in his life in the last 3 months and experiencing frequent stress in his daily life (p<0.05). As a result, it was determined that the mental well-being of the patients who will undergo surgical operation is at a low level, and the descriptive characteristics of the patients and their perceived stress levels have effects on their mental well-being. In line with these results, it is recommended to increase the awareness of nurses about the psychosocial factors affecting the mental well-being of patients in the preoperative period and to include preoperative psychosocial assessment in routine nursing practices.
