Primer sjögren sendromu ile hastaların uyku kalitesi arasındaki ilişki

Özet
Primer Sjögren sendromunun ekstra glandüler belirtileri arasında olan psiko-bilişsel sorunlarda bulunur. Bunlar uyku bozukluğu, depresyon, anksiyete, yorgunluk şeklinde karşımıza çıkmaktadır. Bu çalışmada katılımcıların uyku bozukluğunun tespiti ve bunlara etki eden faktörleri psiko-bilişsel ve hastalık aktivitesi olarak ele aldık. Bu çalışmayı yaparken katılımcılara psiko-bilişsel etki için Beck Depresyon Ölçeği ve Beck Anksiyete Ölçeği; hastalık aktivitesi için EULAR Sjögren Sendromu Hastalık Aktivitesi Skoru,EULAR Sjogren Sendrom Patıent Reported Index, Health Assessment Questionnaire, ve uyku için Pittsburgh Uyku Kalitesi İndeksi uygulandı. C?alıs?mamıza, Trakya U?niversitesi Hastanesi romatoloji poliklig?ine Nisan 2020- 2022 tarihleri arasında bas?vuran, 2016 ACR/EULAR tanı kriterlerine go?re tanısı konulan hastalardan çalışma kriterlerimize uygun 106 hasta alınmıs?tır. Pittsburgh Uyku Kalitesi İndeksi skoruna göre kötü uyku kalitesine sahip hastaların oranı %62,3 saptanmıştır. Yapılan logistik multivariant analizine göre hastaların kötü uyku kalitesini bag?ımsız prediktif etki edenler hastaların depresyon, anksiyete düzeylerinin fazla olmasıdır. Ayrıca literatürde ilk kez albümin düzeylerinin de düşük olması uyku kalitesini olumsuz etkileyen bağımsız etki eden faktör olduğu go?ru?ldu?. Pittsburgh Uyku Kalitesi İndeksi' ne göre uyku kalitesi kötü hastalarda iyi uyku kalitesi olanlara göre komorbid hastalık, EULAR Sjögren Sendromu Hastalık Aktivitesi Skoru; organ tutulumu; EULAR Sjogren Sendrom Patıent Reported Index' nin alt birimi olarak yorgunluk parametresi anlamlı yüksek çıkmıştır. Bunun yanında Health Assessment Questionnaire, ve EULAR Sjogren Sendrom Patıent Reported Index' nin alt birimi olarak ağrı ve kuruluk parametresi uyku kalitesini etkilemedig?i go?ru?ldu?. Sjögren hastaların anksiyete, depresyon, yorgunluk gibi klinik durumların sistemik ve bütüncül olarak ele alınması gerekmektedir. PSS hastalarında uyku bozukluğunun etkilerini araştırmak için daha fazla hastanın dahil edildiği prospektif çalışmalara ihtiyaç vardır.
The extra-glandular manifestations of Primary Sjögren's Syndrome often include psychocognitive problems. These manifest as sleep disorders, depression, anxiety, and fatigue. In this study, we aimed to identify sleep disturbances in participants and factors influencing them in terms of psychocognitive aspects and disease activity. During this study, participants were assessed using the Beck Depression Scale and Beck Anxiety Scale for psychocognitive impact, the EULAR Sjögren's Syndrome Disease Activity Score, EULAR Sjogren's Syndrome Patient Reported Index, Health Assessment Questionnaire for disease activity, and the Pittsburgh Sleep Quality Index for sleep. We included 106 patients who met the study criteria from those who applied to the rheumatology outpatient clinic of Trakya University Hospital between April 2020 and 2022 and were diagnosed according to the 2016 ACR/EULAR diagnostic criteria for Sjögren's Syndrome. According to the Pittsburgh Sleep Quality Index score, 62.3% of patients were found to have poor sleep quality. According to logistic multivariate analysis, the independent predictive factors for poor sleep quality in patients were found to be high levels of depression and anxiety. Additionally, for the first time in the literature, it was observed that low albumin levels were an independent factor negatively affecting sleep quality. In patients with poor sleep quality according to the Pittsburgh Sleep Quality Index, comorbidity, EULAR Sjögren's Syndrome Disease Activity Score, organ involvement, and the fatigue parameter as a subunit of the EULAR Sjogren's Syndrome Patient Reported Index were significantly higher compared to those with good sleep quality. Furthermore, it was observed that the Health Assessment Questionnaire and the pain and dryness parameters as subunits of the EULAR Sjogren's Syndrome Patient Reported Index did not affect sleep quality. Patients with Sjogren's Syndrome clinical conditions such as anxiety, depression, and fatigue must be addressed systemically and holistically. Prospective studies including more patients are needed to investigate the effects of sleep disturbance in pSS patients.
