Konjenital hipotiroidi tanısı alan çocuk ve ergenlerde eşlik eden psikiyatrik bozukluklar ve bilişsel fonksiyonların değerlendirilmesi

Özet
Bu araştırmada konjenital hipotiroidi tanısı alan çocuk ve ergenlerde eşlik eden psikiyatrik bozukluklar, bilişsel defisitler, duygusal ve davranışsal sorunlar değerlendirildi.Araştırma grubu olarak, Trakya Üniversitesi Tıp Fakültesi Çocuk Endokrinoloji Bilim Dalı'nda konjenital hipotiroidi tanısıyla takip ve tedavi edilen, 6-16 yaşları arasındaki 40 olgu alındı. Kontrol grubu olarak, Trakya Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Genel Pediatri Polikliniği'ne ebeveynleri tarafından getirilen kronik tıbbi ve nörogelişimsel hastalığı olmayan 40 olgu alındı. Araştırmaya alınan tüm olgulara sosyodemografik veri formu, güçler güçlükler anketi, okul çağı çocukları için duygulanım bozuklukları ve şizofreni görüşme çizelgesi uygulandı. Aynı zamanda tüm olguların bilişsel fonksiyonlarını değerlendirmek amacıyla WISC-R testi kullanıldı. Konjenitalhipotiroidi tanısı olan hastaların %55'i en az bir, % 20'si iki ve üstünde psikiyatrik tanı aldı.Bu hastalara en sık eşlik eden iki psikiyatrik tanı % 22,5 dikkat eksikliği hiperaktivite bozukluğu, %12,5 özgül öğrenme bozukluğu olarak saptandı.Konjenitalhipotiroidi tanısı olan hastalarda toplam IQ, sözel IQ, performans IQ ve tüm alt testlerin puanları kontrol grubuna göreanlamlı olarak düşük bulundu.Konjenital hipotiroidi tanısı olan olguların; duygulanım, akran ilişkileri, davranım, dikkat eksikliği-aşırı hareketlilik ve toplam güçlük puanları kontrol grubuna göre daha yüksek bulundu. Sonuç olarak tedaviye erken başlanılmasına rağmen konjenital hipotiroidi grubunda bilişsel fonksiyonlarda etkilenme olduğu düşünüldü. Konjenital hipotiroidi grubunda mental retardasyondan ziyade öğrenme ve dikkat problemleri daha sık saptandı. Literatürdeki diğer çalışmaların aksine, dışa yönelim sorunları, içe yönelim sorunlarından daha fazla bulundu.Bu nedenlerle konjenital hipotiroidi tanısı olan olguların bilişsel fonksiyonlar, eşlik edebilecek psikiyatrik bozukluklar ve duygusal davranışsal sorunlar açısından değerlendirilmesinin önemli olduğu kanaatine varıldı. In this study, psychiatric disorders, cognitive deficits, emotional and behavioral problems were evaluated in children and adolescents diagnosed with CH. 6-16 years old, 40 cases being referred to Trakya University Faculty of Medicine, Department of Pediatric Endocrinology that are diagnosed congenital hypothyroidism with who were receiving treatment, were obtained as the study group.40 cases being referred to Trakya University Faculty of Medicine, Department of Pediatrics with no chronical medical or neurodevelopmental disease were obtained as the control group.Socio-demographic data form, strengths and difficulties questionnaires and Schedule for affective disorders and schizophrenia for school age children – present and lifetime version were applied to all cases.Also WİSC-R were used to all cases for assessment cognitive functions. 55% of the patients who diagnosed with CH have at least one, 20% of them have at least two psychiatric disorder. Two most common comorbidity psychiatric diagnosed were ADHD with 22,5%, SLD with 12,5%.The scores of total IQ, verbal IQ, performance IQ, and all subscales were significantly lower in patients with CH than in the control group.In the cases with CH group, hyperactivity,conduct,emotional, peer relations and total diffuculty scores were found higher than in the control group. Consequently, it was thought that in the group cognitive function were affected even though they started to take medication early. It was determined that learning difficulties and attention problems were frequently in the case group rather than mental retardation. Contrary to other studies in the literature, externalizing problems were found were more than internalizing problem. Because of these reasons it's important that assesment cognitive functions, psychiatric disorder, emotional-behavioral problems in the case with CH.
