Ülseratif kolit olgularında inflamatuvar aktivite ile trombin oluşum testi arasındaki ilişki

Özet
İnflamatuvar barsak hastalıkları; etyolojisi kesin olarak bilinmeyen, alevlenmeler ve remisyonlarla seyreden gastrointestinal kanalın kronik patolojileridir. Bu grup içinde yer alan ülseratif kolit; lokal ve sistemik inflamasyonun yanı sıra trombotik olayların artmış riskiyle karakterizedir. Çalışmamızda ülseratif kolit olgularında inflamatuvar aktivite ile tromboza yatkınlık arasındaki ilişkiyi Trombin Oluşum Testi ile göstermeyi amaçladık. Çalışmamıza Trakya Üniversitesi Tıp Fakültesi, İç Hastalıkları Anabilim Dalı, Gastroenteroloji Bilim Dalı'nda takip edilen klinik, endoskopik, histopatolojik bulgularla ÜK tanısı almış 33'ü aktif dönemde, 28'i remisyonda, 18-65 yaş arasında olan 61 hasta ve kontrol grubu olarak 36 sağlıklı gönüllü dahil edildi. Hastalar Mayo inflamatuvar aktivite skorlaması kullanılarak aktif ve remisyonda olarak gruplandı. Çalışmaya alınan olgulardan inflamasyon ve pıhtılaşma parametreleri ile trombin oluşum testi çalışıldı. Aktif dönemdeki ülseratif kolit hastalarının high sensitive C Reaktif Protein, fibrinojen, D Dimer, protrombin zamanı, lag time, peak, start tail düzeyleri arasında remisyon dönemindeki ülseratif kolit hastaları ve kontrol grubuna göre anlamlı fark saptandı. Endojen trombin potansiyeli düzeyleri arasında anlamlı fark saptanmazken, kontrol grubundan yüksek bulundu.Çalışmamız ülseratif kolit hastalarında aktivasyondan bağımsız olarak artmış tromboz potansiyelini göstermektedir. Aktif dönemdeki ülseratif kolit hastalarında ise hem kanama hem de tromboz potansiyelinin artmaktadır.Bu durumu ortaya koymada trombin oluşum testi diğer parametrelere göre daha faydalı görünmektedir.
Abstract
Inflammatory bowel diseases, of unknown etiology, are chronic pathologies of gastrointestinal tract with relapses and remissions in course. Within this group, ulcerative colitis, is characterized by increased risk of thrombotic events, as well as local and systemic inflammation.In our study we objected to show a relationship between inflamatory activity and predisposition to thrombosis by using Thrombin Generation Assay in ulcerative colitis cases.61 patients by clinical, endoscopic and histopathological diagnosis of ulcerative colitis of whom 33 in active and 28 in remission phase, in Gastroenterology Division of Trakya University Medical Faculty Internal Medicine Department, and 36 healthy volunteers as control group; whom all, age ranged between 18-65 were included into our study. Patients were grouped as active and remission by Mayo inflammatory activity score. Inflammation and coagulation parameters and thrombin generation tests were studied in patients and healthy control group, included into study.Statistically significant differences were determined between active phase ulcerative colitis patients` -high sensitive C Reactive Protein, fibrinogen, D-Dimer, prothrombin time, colitis patients` -high sensitive C Reactive Protein, fibrinogen, D-Dimer, prothrombin time, 56 lag time, peak, start tail levels and patients on remission and control group. Endogenous thrombin potential levels were not significantly different, although higher in control group.Our study showed increased thrombosis potential in ulcerative colitis patients independent of activation. On the other hand, both bleeding and thrombosis potential were increased in patients with ulcerative colitis on active phase. Thrombin generation assay seemed more effective than other parameters to reveal this situation
