Sağlıklı bireylerde kardiyorespiratuvar fitnes düzeyinin vücut kompozisyon analizi ve diz kas gücü ile ilişkisi

Özet
Sağlıkla ilgili fitnes (zindelik) bileşenlerinin en önemlisi, düşük seviyesi mortalite ile ilişkili bulunan kardiyorespiratuvar fitnestir. Kardiyorespiratuvar fitnes; süreğen bir fiziksel aktivite boyunca, dolaşım ve solunum sisteminin iskelet kaslarına oksijen sunabilme yeteneğidir. Esas olarak VO2 max (ml/kg/dk) ile ifade edilir. Bu çalışmada, bisiklet ergometride saptadığımız bireye özgü kardiyorespiratuvar fitnes düzeyi ile, vücut kompozisyonu ve diz kas gücü arasındaki ilişkiyi ortaya koymayı amaçladık. Çalışma 18-65 yaş arası, akut/kronik hastalığı ve ilaç kullanımı olmayan 113 sağlıklı gönüllü bireyde yapıldı. Bireylere tartı üzerinde bilgisayarlı vücut kompozisyon analizi ve solunum fonksiyon testi yapıldı. 6 dakika yürüme testi uygulandı. Bisiklet ergometri test ve rehabilitasyon sisteminde maksimal egzersiz testine alınarak metabolik eşlenik ve maksimum watt/kg’ı saptanan, VO2 max düzeyi de indirekt olarak ölçülen bireylerin; sonrasında izokinetik test ve egzersiz sisteminde her iki diz kas kuvveti ve dayanıklılığı ölçüldü. 6 dakika yürüme mesafesinin maksimal egzersiz testi sonucuyla korele olduğunu ve kardiyorespiratuvar fitnes düzeyinin tüm bireylerde vücut yağ yüzdesi ile negatif, yağsız vücut kütlesi ve diz kas gücü değerleri ile pozitif yönde anlamlı ilişkisi olduğunu bulduk. Çalışmaya katılan olguların hiçbirinde, egzersiz testinde komplikasyon yaşanmadı. Çalışma sonucunda, sağlıklı bireylerde kardiyorespiratuvar fitnes düzeyinin, vücut kompozisyonu ve diz kas gücü ile ilişkili olduğu saptandı. Bu durum, vücut yağ yüzdesini azaltmanın, diz kas gücünü geliştirmenin, düzenli aerobik aktivite yapmanın kardiyorespiratuvar fitnes düzeyini artıracağını düşündürür. Sağlıklı bireylerde maksimal aerobik kapasiteyi ölçmek için bisiklet ergometrinin güvenli bir seçenek olduğunu düşünmekteyiz.

The most important component of fitness is cardiorespiratory fitness, whose critical levels are associated with mortality. Cardiorespiratory fitness is the ability of respiratory and cardiac systems to provide the skeletal muscles with oxygen during continuous exercise. It is expressed with VO2max (ml/kg/min). In this study, we aimed to set forth the relation between personal cardiorespiratory fitness level and body composition as well as muscle strength, which were acquired by bicycle ergometry. The research was done with 113 healthy volunteers between the ages of 18 and 65. The individuals underwent computerized body composition analysis on weighing scale and pulmonary function test, as well as six-minutes' walk test. Maximal exercise testing with bicycle ergometry test and rehabilitation system was conducted for the determination of metabolic equivalent, maximum watt/kg and indirect VO2max levels. Bilateral knee muscle strength and endurance were measured with isokinetic testing and exercise systems. We found out that six-minute walk test was correlated to the maximal exercise testing result and cardiorespiratory fitness level in all individuals was positively correlated to fat-free mass and knee muscle strength, negatively correlated to body fat percentage. No complications were encountered during the exercise test. Eventually, cardiorespiratory fitness level of healthy individuals was found to be related to body composition and knee muscle strength. We think that decreasing the body fat percentage,increasing knee muscle strength and doing regular aerobic exercises, increases the cardiorespiratory fitness level. Bicycle ergometry is a safe option to measure the maximum aerobic capacity.
