Sistemik sklerozisli hastalarda uyku bozuklukları

| dc.description.abstract | Sistemik sklerozisli hastalarda uyku problemleri ile ilgili çalışmalar sınırlıdır. Bu çalışmada sistemik sklerozisli hastalarda uykuda solunum problemlerini ve bunların akciğer tutulumu ile olan ilişkisini araştırmak amaçlandı. Çalışmaya Trakya Üniversitesi Romatoloji kliniğinde takip edilen 37'si kadın 39 hasta dahil edildi. Tüm hastalara tomografi, solunum fonksiyon testleri, karbonmonoksit difüzyon testi, ekokardiyografi ve uyku labaratuarında tüm gece polisomnografi yapıldı. Obstruktif sleep apne için apne hipopne indeksi ?5 olarak kabul edildiğinde 21 hastada (%53.8) obstruktif sleep apne mevcuttu. Obstruktif sleep apne olan ve olmayan grup arasında steroid kullanımı, tomografi tutulumu, Warrick skoru, Medsger skalası, pulmoner hipertansiyon olup olmaması açısından anlamlı fark saptanmadı. Apne hipopne indeksi ile yaş (p=0.001; r =0.497) ve beden kitle indeksi (p=0.0001, r=0.57) arasında anlamlı bir korelasyon saptandı. Ortalama uyanıklık saturasyonu ile uyku saturasyonu arasında korelasyon saptandı (p=0.0001; r=0.89). Hem uyanıklık saturasyonu hem de uykudaki ortalama saturasyon ile sistolik pulmoner arter basıncı korele bulundu (sırasıyla p=0.001; r =-0.59; p=0.003, r=-0.55). Uykuda saturasyonun <90 olduğu süre ile sistolik pulmoner arter basıncı korele bulundu (p=0.004; r =0.45). Sistemik sklerozis gibi pulmoner hipertansiyonun nispeten sık görüldüğü bir hastalıkta uykuda solunum bozukluklarının erken tespitinin ve tedavisinin hastalığın progresyonunu yavaşlatabileceğini umut etmekteyiz. | en_US |

| dc.description.abstract | It is a few number of studies about systemic sclerosis in patients with sleep problems. We aimed to investigate the relationship between sleep-disordered breathing and pulmonary involvement in patients with systemic sclerosis in this study. Trakya University of Rheumatology clinical follow-up of 39 patients, 37 of them are women, were included in the study. All patients was performed tomography, pulmonary function tests, carbon monoxide diffusion test, echocardiography and polysomnography in sleep laboratory all night. When we accepted obstructive sleep apnea for apnea hypopnea index ?5; 21 patients (53.8%) had obstructive sleep apnea. There are not significant differences between the groups with obstructive sleep apnea and without obstructive sleep apnea in terms of streoid use, tomography involvement, Warrick score, Medsger scale and pulmonary hypertension. There was found a significant correlation between apnea hypopnea index with age (p=0.001; r=0.497) and body mass index (p=0.0001, r=0.57). There was a correlation between average wake saturation and sleep saturation (p=0.0001; r=0.89). There was found correlation between systolic pulmonary artery pressure and both wake saturation and sleep saturation (respectively p=0.001; r=-0.59 and p=0.003, r=-0.55). There was correleted between duration in sleep saturation <90 and systolic pulmonary artery pressure (p=0.004; r=0.45). Likewise systemic sclerosis, we hope that in a disease which is common relatively seen pulmonary hypertension can slow the progression to detect sleep disorders and treat early. | en_US |
