| dc.description.abstract | ÖZET Bu klinik çalışmada, omurga cerrahisinde donör saha üzerine hemostatik ajanların etkileri incelendi. Hemostatik ajan olarak bone wax ve spongostan kullanıldı. Çalışmada hastalar; bone wax uygulanan hastalar(Grup 1), spongostan uygulanan hastalar(Grup 2), spongostan uygulanıp-alman hastalar(Grup 3) ve hemostatik ajan uygulanmayan kontrol grubu(Grup 4) hastalar şeklinde 4 gruba ayınldı. Grupların seçiminde, hastaların yaşı, cinsi, tanısı ve insizyonun şekli dikkate alınmamıştır. Yaklaşık olarak postoperatif, 1 yıl takip edilmişlerdir. Hastaların hematom (mayi) miktarları kapalı emici drenaj sistemleri ile takip edilmiştir. Drenaj sistemi 48 saat takip edilip sonlandınlmış, yalnız drenaj miktarı 30 cc' nin altına düşmeyen hastalarda sonlandınlmamıştır. Profilaktik olarak, drenaj sistemi sonlandırılıncaya kadar antibioterapi uygulanmıştır. Preoperatif, peroperatif ve postoperatif olarak tüm hastalar izlenmiştir. Sonuçta, hemostatik ajan olarak bone wax uygulanan(Grup 1) ve spongostan uygulanıp-alman hastalarda, non-parametrik varyans analizine(Kruskal-Wallis yöntemi) göre, anlamlı istatistiksel veriler elde edilmiştir. Bone wax ve spongostan oldukça güvenli ve etkili hemostatik ajanlardır. Hiçbir hastada enfeksiyon, ödem ve ağrı gibi komplikasyon gelişmemiştir. Kanayan kansellöz kemik yüzeylerine hemostatik ajan uygulanması komplikasyonların gelişimini engellemektedir. Sonuç olarak, hem bone waxin, hemde spongostanın kanayan kansellöz kemik yüzeylerine uygulanması önerilmiştir. 48 | en_US |
| dc.description.abstract | SUMMARY THE COMPARISON OF EFFECTS OF HAEMOSTATIC AGENTS WHICH ARE USED IN SPINAL SURGERY ON GRAFT DONOR SITE In this study the effects of haemostatic agents on donor site in spinal surgery were investigated. Bone wax and gelfoam(spongostane) were used as haemostatic agents. Patients were divided into 4 groups according to using haemostatic agents; bone wax(Group 1), gelfoam(Group 2), gelfoam apply and remove(Group 3) and no agent (Group 4). The age, sex and diagnosis of the patients and shape of the incision were not taken into consideration during the selection of patient groups. The average follow-up period was 1 year. Closed suction drainage systems were used for evaluating of amount of drainage. The drainage system was used for 48 hours, but the system did not remove in patients with drainage was above 30 cc daily. Antibiotherapy was applied to patients until the drainage system were removed. As a result, statistically meaning full datas were found in the patient group to whom bone wax was applied(Group 1) and to whom gelfoam was applied and then removed(Group 2), with non-parametric varians analysis(Kruskal-Wallis method). Bone wax and gelfoam are reliable and effective haemostatic agents. We haven't seen any complications, such as infection, edema and pain. Application of haemostatic agents to the bleeding cancellous bone surfaces prevents complications. As a result, we suggested the application of bone wax and gelfoam to the bleeding cancellous bone surfaces at donor sites. 49 | en_US |
