Trakya Üniversitesi Sağlık araştırma ve Uygulama Merkezi erişkin acil servisine 112 ambulansı ile yapılan başvuruların değerlendirilmesi

Özet
Trakya Üniversitesi Sağlık Eğitim Merkezi Erişkin Acil Servisi’ne 1 Ocak 2015 ile 1 Ocak 2016 tarihleri arasında Edirne 112 Komuta Kontrol Merkezine bağlı ambulanslar ile olay yerinden tarafımıza getirilen nontravmatik 18 yaş üstü hastalar tespit edilerek incelenmiştir. Hastaların bilgilerine 112 Komuta Kontrol Merkezi kayıtlarından ulaşılmış, kriterlere uygun 1686 vaka tespit edilmiş. Bu vakalar yaş, cinsiyet, getiriliş yeri, getiriliş saati, sosyal güvenceleri, ambulans talep etmek için beyan ettikleri şikayetleri, tansiyon, nabız, cilt rengi, bilinç durumları, solunum paternleri, pupil genişlikleri, şikayetlerin sistemlere göre dağılımı, ön tanıları, son tanıları tespit edildi. Yapılan araştırmada hastaların genelinin kent merkezinden mesai saatleri içinde getirildiği, getirilen hastaların yaş ortalamasının 65 yaş üzeri olduğu, getirilen hastaların genelinin vital bulgularının normal olduğu, büyük kısmının SSK emeklisi olduğu, şikayetlerin genelde kardiyak ve nörolojik patolojilerden kaynaklandığı, tekrar yapılan tetkikler neticesinde hastaların tespit edilen son tanılarının nöroloji ve kardiyoloji kökenli olduğu saptanmıştır. Ekiplerinin ön tanı belirlemekten genellikle imtina ettikleri, düşündükleri ön tanılardan nörolojik ve kardiyolojik olanlarda son tanılarla bir tutarlılık saptanmıştır.112 Komuta Kontrol Merkeziden ambulans talep eden hastaların genelde birden fazla sistemi ilgilendiren şikayetlerde bulundukları bu yüzden 112 ekiplerinin ön tanı belirlemekten ziyade hastanın öyküsünü kayıt almak durumunda kaldıkları saptanmıştır. Buda ön tanı belirtilmeden Trakya Üniversitesi Tıp Fakültesi Erişkin Acil Servise getirilen hasta sayısının fazla olmasına neden olduğu düşünülmektedir.
The non-traumatic patients over the age of 18 who were taken to the Adult Emergency Service of Trakya University, Healthcare Training Center between January 1, 2015 and January 1, 2016 by ambulances of the 112 Command Control Center from event scenes were determined and evaluated. The data of the patients were obtained from the data registers of the 112 Command Control Center, and 1686 cases that fit the inclusion criteria for the study were determined. The ages, genders, the places where the patients were taken, arrival times, social security statuses, the complaints declared for demanding an ambulance, blood pressure values, pulse values, the color of their skins, conscious statuses, respiration patterns, pupil widths, the distribution of the complaints according to the body systems, pre-diagnoses, final diagnoses of the cases were determined. It was determined that the patients were generally brought from city center within working hours, the mean age of the patients was over 65, the vital findings of them were generally normal, the majority of them were retirees of the SSS (Social Security Service), generally the complaints stemmed from cardiac and neurological pathologies, and the final diagnoses after the tests that were repeated stemmed from neurological and cardiological reasons. It was also determined that the healthcare teams usually avoided prediagnosis, and in several prediagnoses that were made by them, there was a consistency between the pre and final diagnoses in those that were neurological and cardiological. It was also determined that the patients who demanded ambulances from 112 Commend Control Center had complaints that were related with more than one body system, and for this reason, the 112 teams generally avoided prediagnoses and merely recorded the history of the patients. It is considered that this situation is the reason for the high number of the patients that are taken to Trakya University, Faculty of Medicine, Adult Emergency Service without prediagnoses.
