Edirne ili 112 acil sağlık hizmetleri istasyonlarında görevli yardımcı sağlık personelinin çocuk istismarına yönelik bilgi tutum ve farkındalıkları

Özet
Çocuk istismarı ve ihmali günümüzde sık rastlanan ve önlenebilir bir toplum sağlığı sorunudur. İstismarın ve ihmalin erken tanısı ve önlenmesi öncelikli hedefler arasındadır. İlk temasın sıklıkla 112 sağlık personeliyle yapılması, çocuklarda erken tanıda oldukça önemlidir. Çalışmamızın amacı çocuk istismarı ve ihmali konusunda 112 sağlık personelinin bilgi, tutum ve davranışını belirlemek ve literatüre bilgi sağlamaktır. Çalışmamız Edirne ilinde görev alan 112 sağlık personeli ile yüzyüze görüşme yöntemiyle yapılan tanımlayıcı bir anket çalışmasıdır. Anketimiz 150 katılımcıyla yapıldı. Ankette katılımcıların sosyodemografik özellikleri, istismar ve ihmal ile ilgili ölçek sorularına yer verildi. Sorular 5 puan üzerinden değerlendirildi. Katılımcıların %62’si kadın; yaş ortalaması 32,02±5,27 yıl; %59,4’ü acil tıp teknisyeni ve %40,6’sı paramedikti. Katılımcıların %73,3’ü evli ve %69’u çocuk sahibiydi. Yine %59’u daha öncesinde çocuk istismarı ilgili eğitim aldıklarını belirtti. Anketin genel ortalama puanı 3,76 ± 0,35 idi. Katılımcılardan erkeklerin istismar ve ihmal konusunda kendilerini kadın çalışanlara göre anlamlı olarak yeterli olarak belirtmelerine rağmen; istismar ve ihmal olgularındaki fiziksel belirtiler ve istismar ve ihmal olgularındaki davranışsal belirtiler ölçeklerinde anlamlı olarak kadınlardan daha düşük ortalama puanlar aldıkları görüldü. İstismar ve ihmal eğitimi daha öncesinde alan katılımcıların anlamlı olarak daha fazla çocuk istismarı ve ihmali bildirdiklerini belirttiler. Çocuk istismarı ve ihmali eğitimi alan kişilerin, evlilerin ve çocuk sahibi olan çalışanların istismar ve ihmal olgularındaki fiziksel belirtiler ve istismar ve ihmal olgularındaki davranışsal belirtiler ölçeklerinde anlamlı yüksek fazla puan ortalamalarına sahip oldukları görüldü. 112’de görevli sağlık çalışanlarının istismar ve ihmalinin fiziksel ve davranışsal belirtilerinde bilgi düzeylerinin yeterli olduğu düşünülse de diğer istismar türlerinde yeterli bilgiye sahip olmadıkları ve bildirim konusunda net bir tutumda olmadıkları görüldü. Belirli zamanlarla yapılacak eğitimlerin bilgi, tutum ve davranışlarda iyi yönlü gelişme sağlayacağını düşünmekteyiz.
Child abuse and neglect is a common and preventable public health problem. Early diagnosis and prevention of child abuse and neglect are among the priority targets. Making the first contact frequently with 112 health care worker is very important in early diagnosis in children. The aim of our study has to determine the knowledge, attitudes and behaviors of 112 health care worker about child abuse and to provide information to the literature. Our study is a descriptive survey study conducted by face-to-face interview method with health care worker working in Edirne province. Our survey is conducted with 150 participants; There are scale questions about the sociodemographic characteristics of the participants and their child abuse and neglect. The questions were evaluated out of 5 points. 62% of the participants were women; mean age 32.02±5.27 years; 59.4% were emergency medical technicians and 40.6% were paramedics. 73.3% of the health care worker were married and 69% had children. Again, 59% stated that they had received training on child abuse before. The overall mean score of the questionnaire was 3.76 ± 0.35. Although the men among the participants stated that they were significantly sufficient compared to the women employees about child abuse and neglect; it was observed that the mean scores of the women were significantly lower than the women in the physical symptoms scales in the child abuse cases and the behavioral symptoms scales in the child abuse cases. They stated that the participants who had previously received the child abuse training reported significantly more child abuse and neglect. It was observed that people who received child abuse and neglect training, married people and employees with children had significantly higher mean scores on physical symptoms in abuse and neglect and behavioral symptoms in abuse and neglect scales. Although it was thought that the level of knowledge of the health workers in 112 on the physical and behavioral symptoms of child abuse was sufficient, it was observed that they did not have enough knowledge about other types of abuse and did not have a clear attitude about reporting. We think that the trainings to be held at certain times will provide a positive development in knowledge, attitudes and behaviors.
