Kan basıncı ve hematokrit arasındaki ilişkiler

Özet
Hipertansiyon gelişiminde hematokrit yüksekliği ile kan viskositesi artışının olası etkisi çok tartışılmakla birlikte henüz kesin olarak açıklığa kavuşmamıştır. Bu çalışmada, kan basıncı ile hematokrit arasındaki olası ilişki, Trakya Üniversitesi Tıp Fakültesi Hastanesi Endokrinoloji Kliniğinde 2009 ve 2010 yılları arasında yatan 548 olguda (normal veya hipertansiyonlu) retrospektif olarak araştırıldı. Olgular; herhangi bir hipertansiyon ilacı kullanmayanlar (106 kişi), diüretiksiz (30 kişi) ve diüretikli tansiyon ilacı kullananlar (54 kişi) ve sekonder hipertansiyonlular (358 kişi) olarak dört gruba ayrıldı. Tüm olgular birlikte değerlendirildiğinde, yaşın etkisi sabitken hematokrit ile sistolik kan basıncı ve diyastolik kan basıncı arasında istatistiksel olarak anlamlı ilişki saptanmamıştır (p>0,05). Yaşın etkisi sabitken, hipertansiyon ilacı kullanmayan grupta, diüretiksiz ilaç kullanan grupta ve diüretikli ilaç kullanan grupta ayrı ayrı, hematokrit ile sistolik kan basıncı ve diyastolik kan basıncı arasında istatistiksel olarak anlamlı ilişki saptanmamıştır (p>0.05). Sonuç olarak; kan basıncı ve viskoziteyi etkileyen kompansatuvar mekanizmaların yanısıra diğer parametreler dedikkate alınarak homojen gruplarda daha kapsamlı çalışmalara ihtiyaç duyulmaktadır.
Abstract
Probable effects on developing hypertension due to increase in hematocrit level and related increase in whole blood viscosity is still controversial and couldn?t be completely enlighted. In our trial, we investigated probable relationship between blood pressure and hematocrit level retrospectively on 548 patients who had been hospitalized in Endocrinology Clinic in Trakya University Hospital in years between 2009 and 2010. The patients were grouped into four, such as; patients who don?t use any anti-hypertensive drugs (106 people), patients who use anti-hypertensive drugs without diuretics (30 people), patients who use anti-hypertensive drugs combined with diuretics (54 people), patients who has a situation that causes secondary hypertension (358 people). When the ages were stable, we evaluate the correlation between hematocrit levels and systolic blood preesure and diastolic blood pressure. When we evaluate patients all together, there were statistically no significant correlation between hematocrit levels and systolic blood pressure and diastolic blood pressure (p>0.05). When we evaluate each group seperately; patients who don?t use any anti-hypertension drugs, patients who use anti-hypertensive drugs without diuretics, patients who use anti-hypertensive drugs combined with diuretics, there were statistically no significant correlation between hematocrit levels and systolic blood pressure and diastolic pressure (p>0.05). As a conclusion, beside compensatory mechanisms that effect blood pressure and hematocrit levels, other parameters must also be taken in consideration and further studies is needed to enlighten those mechanisms and relationships between blood pressure and hematocrit levels in more homogeneous groups and in more extensive investigations.
