Mitral kapak prolapsusu olan çocuk ve ergenlerde psikiyatrik değerlendirme ve bakım verenlerinin stresle başa çıkma tarzlarının incelenmesi

Özet
Çalışmamızda mitral kapak prolapsusu olan çocuk ve ergenlerde psikiyatrik belirti ve bozukluklar, bakım verenlerinin kaygı düzeyleri ve stresle başa çıkma tarzları değerlendirilmiş, sağlıklı kontrollerle karşılaştırılmıştır. Araştırma grubu olarak, Trakya Üniversitesi Tıp Fakültesi Çocuk Kardiyoloji Bilim Dalı’nda, mitral kapak prolapsusu tanısıyla takip ve tedavi edilen 76 olgu alınmıştır. Kontrol grubu olarak, Trakya Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Genel Pediatri Polikliniği’nde değerlendirilen çalışmamızın dahil edilme kriterlerini karşılayan 76 olgu alınmıştır. Çalışmamızdaki tüm katılımcılara Okul Çağı Çocukları için Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Versiyonu – Mental Bozuklukların Tanısal ve Sayımsal El Kitabı 5-Türkçe Uyarlaması, durumluk-sürekli kaygı ölçeği ya da çocuklar için durumluluk-sürekli kaygı ölçeği, 11 yaş ve üzeri olgular için güçler ve güçlükler anketi-ergen formu uygulanmıştır. Tüm olguların bakım verenlerine sosyodemografik veri formu, güçler güçlükler anketi, durumluk-sürekli kaygı ölçeği, stresle başa çıkma tarzı ölçeği uygulanmıştır. Mitral kapak prolapsusu olan olguların %88,2’sinin yaşam boyu en az bir psikiyatrik tanı aldıkları saptanmıştır. Mitral kapak prolapsusu olan olgularda yaşam boyu panik bozukluk, ayrılık anksiyetesi bozukluğu, enürezis, sınav kaygısı tanı oranlarının kontrol grubuna kıyasla daha yüksek olduğu tespit edilmiştir. Mitral kapak prolapsusu olan olguların bakım verenlerinin kendine güvenli ve sosyal destek arama yaklaşımı skor ortalamalarının kontrol grubundan anlamlı düzeyde daha yüksek olduğu bulunmuştur. Mitral kapak prolapsusu olan olguların bakım verenlerinin başa çıkma tarzları ile olguların bazı ruhsal belirtileri arasında ilişki olduğu görülmüştür. Çalışmamızın sonuçları, mitral kapak prolapsusu olan olgularda anksiyete bozukluklarının daha sık görüldüğünü, bakım verenlerinin başa çıkma tarzları ile olguların ruhsal belirtileri arasında karşılıklı etkileşim olduğunu göstermiştir.
In our study, psychiatric symptoms and disorders in children and adolescents with mitral valve prolapse, anxiety levels and coping styles of their caregivers were evaluated and compared with healthy controls. 76 patients who were followed up and treated with the diagnosis of mitral valve prolapse in Trakya University Faculty of Medicine, Department of Pediatric Cardiology were included as the research group. 76 patients who met the inclusion criteria of our study, that were evaluated in Trakya University Faculty of Medicine, Pediatrics General Pediatrics Polyclinic, were included as the control group. Affective Disorders and Schizophrenia Interview Schedule-Now and LifetimeVersion- Diagnostic Statistical Manual 5-Turkish Version for School-Age Children, state-trait anxiety scale, strengths and difficulties questionnaire-adolescent form forcases 11 years and older were applied to all participants evaluated in the study. Sociodemographic data form, strengths and difficulties questionnaire, state-trait anxiety scale and ways of coping inventory were applied to caregivers of all cases. It was determined that 88,2% of the cases with mitral valve prolapse received at least one psychiatric diagnosis throughout their lifetime. Additionally the diagnosis rates of lifetime panic disorder, separation anxiety disorder, enuresis and test anxiety were higher in cases with mitral valve prolapse compared to the control group. It was found that the caregivers of the participants with mitral valve prolapse had a significantly higher mean score on social support seeking and self confident approach compared to the control group. It has been observed that there is a relationship between the copingstyles of the caregivers of the patients with mitral valve prolapse and some psychological symptoms of the patients. The results of our study showed that anxiety disorders are more common in patients with mitral valve prolapse and that there is a mutual interaction between the caregivers' coping styles and the psychological symptoms of the patients.
