The value of nizatidine and famotidine in the prophylaxis of acid aspiration pneumonia in cesarean operation

The effect of nizatidine, famotidine and placebo in decreasing the risk of acid aspiration pneumonia were investigated in 75 pregnants undergoing elective cesarean operation and their 75 neonates. Group A consisted of pregnants divided into three subgroups. Subgroup I were given nizatidine 300 mg per os, 90 minutes prior to anaesthetic induction and groups II and III were given famotidine 40 mg per os and saline (control) 10 mL respectively. Gastric contents of pregnants after intubation and before extubation, and of neonates after being delivered were aspirated through the nasogastric tube and pH and volume of gastric contents were measured. Maternal and umblical arterial blood gase samples were taken from both pregnants and neonates respectively to get analysed. Apgar scores at first and 5 th minutes in neonates were recorded. pH gastric content of pregnants after intubation and before extubation, was significantly increased but its volume decreased in nizatidine and famotidine groups when compared to the placebo ones. Although 14 (56 %) cases after intubation and 1 (4 %) case before extubation, both being in control subgroup, were at risk for the acid aspiration pneumonia (pH ? 2.5 and volume ? 25 mL) but on the other hand, in nizatidine and famotidine subgroups no risk were present. Results of blood gases samples in pregnants and their neonates were formal to be no significant in three, groups respectively. pH and volume gastric content and Apgar scores values in neonates in three groups were also insignificant at related times. As a result, two drugs used were concluded to be equally effective in the prophylaxis of acid aspiration pneumonia.
