Özet
Trakya üniversitesi Sağlık Araştırma ve Uygulama Merkezi Acil Servisi'ne başvurup hematoloji konsültasyonu istenen hastaların demografik verilerini, vital bulgularını, laboratuvar sonuçlarını araştırdık. 1 Ocak 2016 – 31 Aralık 2018 tarihleri arasında acil servisimize başvuran ve hematoloji konsültasyonu istenen hastalar geriye dönük olarak hastane otomasyon sistemi ve hasta dosyalarından incelendi. Çalışma kriterlerine uygun 428 başvuru kaydına ulaşıldı. Hastaların acil servis başvurularındaki vital bulguları, başvuru şikayetleri, laboratuvar parametreleri, demografik özellikleri, acil servis takip süreleri, yatış ya da taburculuk durumları incelendi. Toplanan veriler istatistiksel olarak incelendi. Hastaların %57,2'si erkekti. Hastaların yaş ortalaması 61±19,1 yıl olarak tespit edildi. Hastaların %36,4'ü hastaneye ambulans hizmetleri ile başvurdu. Hastaların %52,5'inde en az bir adet eşlik eden hastalık mevcuttu. Hastalarda en sık halsizlik (%20,6) ve ateş (%15,0) şikayeti gözlendi. Muayene bulgularında; artan kalp tepe atımı ve solunum sayısının yatış üzerine anlamlı etkisinin olduğu fakat takip süresine etkili olmadığı gözlendi. Bozulmuş mental durumun yatış ihtmalini 5,5 kat arttırdığı görüldü. Hastaların platelet değerlerindeki artışın yatış ihtimalini azalttığı gözlendi. Hastaların %34,1'ine bir ya da birden fazla kan transfüzyonu yapıldığı gözlendi. Takip sonrasında hastalar %50,93 oranında hematoloji kliniğine yatırıldı. Çalışmamıza benzer şekilde literatürde kapsamlı bir çalışma tespit edilemedi. literatürde, incelenen veriler tekli şekilde araştırıldığında bazı değerlerin benzer bazılarının ise farklı olduğu gözlendi. Bu konuda ileride yapılacak çalışmaların literatüre katkı sağlayacağı, çalışma yaptığımız konuya ise açıklık getireceğine inanmaktayız.
We investigated demographic datas, vital signs, laboratory test results of the patients who applied Trakya University Health Research and Application Center Emergency Department and consulted haematology clinc afterwards. We retrospectively analyzed patients who applied our emergency room and consulted haematology department between the dates of January 1 2016 – December 31 2018 from patient files in archive and hospital automation system. 428 application records have been reached in accordance with the study criteria. Vital signs, complaints, laboratory parameters, demographic features, follow up period in ER, hospitalization or discharge status of the patients were evaluated and these collected datas were statistically analyzed. %57.2 of patients were male. The mean age of the patients was 61 ±19,1 years. %36.4 of patients applied to the hospital by using ambulance service. %52.5 of the patients had at least one concomitant disease. The most common complaints were weakness (%20,6) and fever (%15,0) among patients. Examination findings showed that increased heart beat and respiration rate had significant effect on hospitalization but did not affect follow up period. Impaired mental status increased the likelihood of hospitalization by 5,5 times. It was observed that increase platelet levels decreased the likelihood of hospitalization. %34,1 of patients were had blood transfusion once or more than once. After the follow up period, %50,93 of the patients admitted to the haematology department. Similar to our study, no comprehensive study was found in the literature. In the literature, when the data examined one by one, although some results were similar but others were different. We believe that future studies on this subject will contribute to the literature and clarify the subject.
