Hidradenitis süpürativalı hastalarda dokuda interlökin-6, interlökin-8 ve tümör nekroz faktör benzeri zayıf apopitoz indükleyici (tweak) ekspresyonlarının değerlendirilmesi

Özet
Hidradenitis süpürativa, ağrılı nodül ve abselerle seyreden, çift komedonların karakteristik olarak görüldüğü, ilerleyen evrelerde sinüs ve skatrislerin oluşabildiği kronik inflamatuar bir hastalıktır. Ağrı, hareket kısıtlılığı, kötü koku, akıntı gibi sebeplerle hastaların günlük yaşamlarını olumsuz etkilemektedir. Çalışmamızın amacı hidradenitis süpürativa hastaları ile sağlıklı gönüllülerin derilerinde interlökin-6, interlökin-8 ve tümör nekroz faktör benzeri zayıf apopitoz indükleyici gen ekspresyon düzeylerini değerlendirmek ve karşılaştırmaktır. Çalışmaya Trakya Üniversitesi Deri ve Zührevi Hastalıklar Polikliniği'ne başvuran, klinik olarak tanı almış ve tedavisiz olan 19 hidradenitis süpürativa hastası ve 18 sağlıklı gönüllü dahil edilmiştir. Hasta ve sağlıklı gönüllülerin aksilla veya inguinal bölgelerinden 4 mm punch biyopsi alınmıştır. Hastaların biyopsileri lezyondan lezyonel ve lezyon kenarındaki sağlam görünen deriden olmak üzere iki farklı alandan yapılmıştır. Hastaların demografik ve klinik özellikleri kaydedilmiştir. Çalışmamızda hasta grubunda interlökin-6 ve interlökin-8 düzeyi lezyonel alanda sağlıklı gönüllülere göre artmış olarak bulunmuşken tümör nekroz faktör benzeri apopitoz indükleyici düzeyinde anlamlı fark saptanmamıştır. Çalışmamız gelecek çalışmalarda interlökin-6, interlökin-8 ve tümör nekroz faktör benzeri apopitoz indükleyici sitokinlerinin tedavi seçeneği olarak yer almasına ışık tutabilir.
Hidradenitis suppurativa is a chronic inflammatory disorder characterized by the presence of double comedones, painful nodules, abscesses, sinuses, and cicatrices in later stages. Its debilitating symptoms, including pain, restricted mobility, malodor, and discharge, significantly impair patients' quality of life. The primary objective of our investigation is to assess the expression levels of key cytokines, namely interleukin-6, interleukin-8 and tumor necrosis factor like weak inducer of apoptosis, in both hidradenitis suppurativa lesions and unaffected skin regions of healthy individuals. A cohort comprising nineteen clinically diagnosed hidradenitis ssuppurativa patients from the Trakya University Dermatology and Venereology Clinic, alongside eighteen healthy volunteers, was enrolled for the study. Utilizing 4 mm punch biopsies, tissue specimens were obtained from the axillary or groin regions of both patient and control groups. Samples retrieved from hidradenitis suppurativa patients encompassed two distinct sites: the active lesions and adjacent perilesional skin exhibiting no apparent pathology. Comprehensive demographic and clinical profiles of the participants were meticulously documented. Our investigation unveiled a notable upregulation of interleukin-6 expression in both lesional and perilesional areas, alongside heightened interleukin-8 levels specifically within the lesional sites of hidradenitis suppurativa patients, when juxtaposed with those observed in the healthy volunteer cohort. Conversely, no statistically significant variance was discerned in tumor necrosis factor like weak inducer of apopitosis expression levels between the study groups. These findings provide valuable insights into the potential therapeutic utility of individual cytokines, particularly interleukin-6 and interleukin-8, in the management of hidradenitis suppurativa.
