Düşük doz diode lazerin gingivektomi/plasti sonrası yara iyileşmesi üzerine etkinliğinin histopatolojik olarak değerlendirilmesi

Özet
Bu çalışmanın amacı kronik enflamatuvar dişeti büyümesi görülen bireylerde gingivektomi/plasti operasyonları sonrasında düşük doz diode lazer (DDDL) tedavisinin yara iyileşmesine olan etkisini histopatolojik olarak değerlendirmektir. Çalışmamıza sistemik olarak sağlıklı (19-56 yaş) kronik enflamatuvar dişeti büyümesi tanısı konulmuş 28 birey (20 kadın, 8 erkek) dahil edilmiştir. Çalışmaya katılan tüm bireylerin, gingivektomi/plasti sonrası rastgele seçilen yarım çenelerinin bir tarafını test grubu, diğer tarafını kontrol grubu oluşturmuştur. Test grubuna işlemden hemen sonra ve 1.,3., 7., günlerde düşük doz diode lazer uygulanmıştır. Çalışmaya dahil edilen tüm hastaların periodontal indeks ölçümleri (plak indeksi, gingival indeks, sondlamada kanama, sondlanabilir cep derinliği) başlangıçta, başlangıç periodontal tedavi (BPT) sonrası 2. haftada ve operasyon sonrası 21. günde yapılmıştır. Hastalardan operasyondan önce ve operasyon sonrası 1., 3., 7. ve 21. günde alınan sürüntü örnekleri histopatolojik olarak süperfisyal hücre indeksleri (SHI) ve keratinizasyon indeksleri (KI) kullanılarak değerlendirilmiştir. Klinik periodontal ölçümlerinde 14. ve 21. günde elde edilen değerler, başlangıç değerlerine göre anlamlı derecede azalma göstedi (p<0,001). Gingivektomi/plasti sonrası değerlendirilen keratinizasyon ve süperfisyal hücre indeksleri grup içi analizde 1. gün azalırken 3., 7. ve 21. günde hem kontrol grubunda hem de test grubunda anlamlı derecede artış gösterdi (p<0,001). Yapılan gruplar arası karşılaştırmada ise başlangıç, 1., 3., 7. ve 21. gün keratinizasyon indeksi ve süperfisyal hücre indeksi değerlerinde, kontrol grubu ile test grubu arasında anlamlı fark saptanmadı. Çalışmamıza göre düşük doz diode lazerin gingivektomi/plasti sonrası yara iyileşmesi üzerinde belirgin bir etkisinin olmadığı görülmüştür. The aim of this study is to evaluate histopathologically the effect of low level diode laser treatment on wound healing after gingivectomy/plasty operations in patiens with chronic inflammatory gingival growth. Our study included 28 patients (20 females, 8 males who were systemically diagnosed with healthy (19-56 years old chronic inflammatory gingival growth. Randomly selected half quadrants of all patients participating in the study after gingivectomy/plasty constituted the test group in one areas and the control group in the other. Low level diode laser was applied to the test group immediately after the procedure and on the 1st, 3rd, and 7th days. Periodontal index measurements (plaque index, gingival index, bleeding on probing, probing pocket depth) of all patients included in the study were performed at the beginning, at the 2nd week after the initial periodontal treatment and on the 21st day after the operation. The samples taken from the patients preoperatively and on the 1st, 3rd, 7th and 21st days after the operation were evaluated histopathologically using the superficial cell indexes and keratinization indexes. In clinical periodontal measurements, the values obtained on the 14th and 21st days showed a significant decrease compared to the baseline values (p<0,001). Keratinization and superficial cell indices evaluated after gingivectomy/plasty decreased on the 1st day in the in-group analysis, while on the 3rd, 7th and 21st days, they increased significantly in both the control group and the test group (p<0,001). In the comparison between the groups, no significant difference was found between the control group and the test group in terms of initial, 1st, 3rd, 7th and 21st day keratinization index and superficial cell index values. According to our study, it was observed that low dose diode laser had no significant effect on wound healing after gingivectomy / plasty.
