Özet
Çalışmamızda öncelikle tip 2 Diabetes mellituslu vakaların iyi ya da kötü kontrollü olması ve diyabetin kronik komplikasyonları (mikroalbuminüri, proteinüri, diyabetik retinopati ve diyabetik nöropati) açısından değerlendirilmesi amaçlandı. Ayrıca çalışmamızda tip 2 Diabetes mellituslu hastalardaki melatonin düşüşünün, sağlıklı olmakla birlikte güçlü bir melatonin baskılayıcı olan ışığa gece periyodunda maruz kalan deneklerdeki melatonin düşüşüyle kıyaslanması amaçlandı.Trakya Üniversitesi Tıp Fakültesi İç hastalıkları Anabilim Dalı, Endokrinoloji ve metabolizma hastalıkları Bilim Dalında yürütülen çalışmaya 35 Tip 2 Diabetes mellitus tanılı hasta ve 15 sağlıklı denek alındı. Tüm tip 2 Diabetes mellituslu hastalardan karanlıkta ve karanlık gerekliliği olmayıp beyaz ışığa maruz kalan tüm sağlıklı bireylerden gece boyunca belirli saatlerde melatonin için kan alındı.Çalışmamıza dayanarak ışığın melatonin sentez ve salınımını, diabetes mellitus ve yaşlanma gibi diğer melatonin seviyesini baskılayıcı etmenlerden çok daha belirgin olarak baskıladığı sonucuna vardık. Diyabetin iyi ve kötü kontrollü olması arasında melatonin salınımı açısından belirgin bir fark saptamadık. Tanı anında mikrovasküler komplikasyonların başlamış olması bozulmuş melatonin salınımını değiştirmemektedir.Elde ettiğimiz verilerin desteklenmesi için gece ışığa maruz bırakılan diyabetiklerle, karanlıkta geceyi geçiren sağlıklı denekler gibi yeni grupların da oluşturulması düşünülebilir.
In our study, we first aimed to evaluate cases of type 2 Diabetes mellitus is controlled for better or worse or have any chronic complications of diabetes. All healthy individuals exposed to white light, not dark necessity limited hours during the night, blood samples were taken for melatonin. Based on our study of melatonin synthesis and secretion of light, such as diabetes mellitus and aging factors other than melatonin levels were significantly suppressed the inhibitory concluded. Between good and bad of diabetes is controlled in terms of melatonin secretion did not observe a significant difference. Impaired microvascular complications of diabetes have begun the time of diagnosis does not change the release of melatonin. Our data is exposed to light at night to support people with diabetes, healthy subjects were spending the night in the dark, such as the addition of new groups that may be considered.
