Özet
Avrupa'da kadınlar arasında kardiyovasküler hastalıklardan en sık ölüm Türkiye'dedir. Bu çalışmanın amacı normal stres gated miyokard perfüzyon sintigrafili kadınlarda kardiyak olay oranını tespit etmektir. Bu retrospektif çalışmaya Haziran 2001- 2005 tarihleri arasında Trakya Üniversitesi Hastanesi Nükleer Tıp bölümüne şüpheli veya bilinen koroner arter hastalığının değerlendirilmesi amacıyla gönderilen ve normal stres gated miyokard perfüzyon sintigrafisi sonucuna sahip ardışık 689 kadın hasta dahil edildi. Prognoz değerlendirilmesi için ulaşılan 333 hastanın kardiyak olay sonuçları kaydedildi. Tüm hastalar için izlem süresi en az 1 yıl olmak üzere ortalama 39.85±14.24 ay idi. Framingham risk skorlarına göre hastaların %55'i düşük risk, %45'i orta risk grubunda idi. İzlemde 5 hastada ağır kardiyak olay, 9 hastada ise hafif kardiyak olay gelişti. Bu hastalardan 12'si orta, 2 tanesi düşük risk grubundaydı. Bu çalışmanın sonuçları; düşük ve orta riskli ve normal miyokard perfüzyon sintigrafili kadınlarda kardiyak olay oranı ilk yıl için %1.8, beş yıl için %4.2 idi. Düşük riskli kadınlarda bu oran ilk yıl için %0.57, beş yıl için %1.14'dü. Orta riskli grupta ise ilk yıl için %1.9, beş yıl için %7.6 olarak hesaplandı. Normal stres gated miyokard perfüzyon görüntüleme kadınlarda prognoz değerlendirilmesinde değerli ve kullanışlı noninvaziv bir yöntemdir. In the Europe, the most frequent death from cardiovascular disease among women is in Turkey. The aim of this study was to determine the cardiac events rates of normal stress gated myocardial perfusion imaging in women. This retrospective study was composed of 689 consecutive female outpatients with normal stress myocardial perfusion imaging between June 2001 and June 2005, at hospital of the Trakya University departman of Nuclear Medicine for evaluation of suspected or known coronary artery disease. Of the 689 patients, 333 patients were examined for cardiac events during the follow up. The follow up duration was at least 1 year. The mean period was 39.85 ± 14.24 months. Clinically 55% of patients had low-risk and 45% of patients had intermediate risk according to Framingham risk scores. On follow up, 5 patients had hard cardiac events, 9 patients had soft cardiac events. Twelve of patients with hard or soft cardiac events had clinically intermediate risk, and 2 patients had low risk. The results of this study showed that cardiac events rates in women who had low and intermediate risk, and normal stress gated myocardial perfusion imaging are approximately 1.8% at the first year, and 4.2% at the 5 year. Cardiac events rates in the women with low risk are 0.57% at the first year, and 1.14% at the 5 year. Cardiac events rates in the women with intermediate risk are 1.9% at the first year and 7.6% at the 5 year.
