Artroskopik ön çapraz bağ rekonstruksiyonunda femoral tespit yöntemlerinin sonuçlarının değerlendirilmesi ve karşılaştırılması

Özet
Bu çalışmanın amacı kliniğimizde artroskopik ön çapraz bağ rekonstrüksiyonu yapılan hastaların klinik sonuçlarının değerlendirilmesi ve farklı femoral tespit yöntemlerinin klinik sonuçlarının karşılaştırılmasıdır. Trakya Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'nda, Ocak 2008 ve Ekim 2012 tarihleri arasında, artroskopik ön çapraz bağ rekonstrüksiyonu yapılan 100 hasta incelendi. Hastaların 95 (%95)'i erkek, 5 (%5)'i kadındı. Hastaların yaş ortalaması 29 idi. En genç olgu 18, en yaşlı olgu 51 yaşındaydı. Çalışmaya alınan hastalar, femoral tespit yöntemine göre `Endobutton, `Transfix ve `Aperfix olmak üzere 3 gruba ayrıldı. Hastaların cerrahi sonrası ayağa kalkış zamanı, ağrı süresi, normal günlük yaşama, meslek hayatına, sportif faaliyetlere ve son diz eklem hareket açıklığına dönüş süresi, ön çekmece ve Lachman testleri, uyluk atrofisi, Uluslararası Diz Dokümentasyon Komitesi subjektif diz değerlendirme skoru, Kısa Form 36 skoru, Lysholm diz skoru, Tegner puanı, revizyon ihtiyacı ve sportif faaliyetlerde etkilenme düzeyi değerlendirilerek klinik sonuçları ortaya kondu. Ardından klinik sonuçlar gruplar arasında istatistiksel olarak karşılaştırıldı. Cerrahi sonrası ön çekmece ve Lachman testlerinde belirgin iyileşme olduğu gözlemlenerek artroskopik ön çapraz bağ rekonstrüksiyonunun diz stabilitesinin sağlanmasında etkili bir yöntem olduğu gösterildi. Yine de hastaların %64?ünün eski sportif faaliyetlerine dönüşte sorun yaşadığı gözlemlendi. `Transfix grubunda cerrahi sürenin ve cerrahi sonrası normal günlük yaşama dönüş süresinin daha kısa olduğu bulundu. Karşılaştırılan diğer faktörler açısından farklı femoral tespit tekniklerinin birbirine üstünlüğünün olmadığı ortaya konuldu.
Abstract
The aim of this study is to evaluate the clinical results of patients operated in our clinic for arthroscopic anterior cruciate ligament reconstruction and to compare the clinical results of different femoral fixation techniques. A hundred patients who underwent arthroscopic anterior cruciate ligament reconstruction in Trakya University Faculty of Medicine Department of Orthopedics and Traumatology between January 2008 and October 2012 were studied. Study group was included of 95 male (%95) and 5 female (%5) patients. Mean age of the patients was 29 (18-51). The patients in the study group were divided into 3 groups according to femoral fixation techniques as `Endobutton, `Transfix and `Aperfix. Mobilization time and duration of painful period after surgery, time to return normal daily life, occupation, sportive activities and last range of motion level, anterior drawer and Lachman tests, quadriceps atrophy, International Knee Documentation Committee subjective knee evaluation score, Short Form 36 score, Lysholm knee score, Tegner point, necessity of revision, success level at sportive activities of the patients were evaluated. Clinical results of 3 groups were compared statistically. Anterior drawer and Lachman tests showed significant recovery after surgery and this finding proved that arthroscopic anterior cruciate ligament reconstruction is an effective treatment modality to provide knee stability. On the other hand it was seen that %64 of patients had some troubles to return back to their previous sportive activities. It was found that the operation time and the time to return normal daily life was shorter in `Transfix group. Other factors compared between groups were statistically similar and different femoral fixation techniques had no advantage to each other.
