Sepsis oluşturulmuş ratlarda kefirin etkinliği

Özet
Kefirin medikal özellikleri arasında antioksidan, antitümör, antiinflamatuvar olması sayılabilir. Ancak sepsisteki koruyucu etkisi halen bilinmemektedir. Bu çalışmanın amacı septik hayvanlardaki organ disfonksiyonunda kefirin koruyucu etkisinin olup olmadığını göstermektir. Ratlar gelişigüzel olarak 4 gruba ayrılmıştır. Kontrol grubuna (grup I; n=7) hiçbirşey verilmemiştir. Yalnız kefir grubuna (grup II, n=7) 2ml rat başına kefir verildi. Diğer ratlara anestezi uygulanarak intraperitoneal Escherichia coli verildi. Enjeksiyondan 5 saat sonra sepsis grubu (grup III, n=7) ve sepsis+ kefir grubu (grup IV, n=7) oluşturuldu. Tüm ratlar 8.günde yüksek doz pentotal verilerek öldürüldü ve orta hat laparatomi yapıldı. Akciğer, karaciğer, böbrek, dalak ve kalınbarsaklar histopatolojik muayene için çıkarıldı, tüm segmentler inflamasyon ve doku değişimi açısından incelendi. Akciğerlerde ödem ve konjesyon, alveol içlerinde polimorf nüveli lökositten zengin mikst tipte iltihabi hücre kümelenmesi, bronş çevrelerinde iltihabi hücre infiltrasyonu ve bazı alanlarda amfizematöz, bazı alanlarda ise atelektatik alanların, sepsis + kefir grubunda az ya da hiç olmadığı gözlendi. Karaciğerde sepsis oluşturulan grupta hepatositlerde geniş yayılımlı hidrofik dejenerasyon görülürken, kefir verilen grupta ise hepatositlerde hidrofik dejenerasyon ve inflamatuvar infiltrasyon, portal alanda görülmedi. Böbrekte proksimal tüplerde epitelyumda nekroz, korteks ve medullada interstisyel alanda inflamatuvar reaksiyon kefir grubunda daha az gözlendi. Dalakta kırmızı pulpada genişleme, kanama, fokal alanlarda mikst tipte iltihabi hücre infiltrasyonu ve konjesyon sepsisli grupta gözlenirken; sepsis + kefir grubunda konjesyon dışında bulgu izlenmedi. Kalınbarsaklarda; sepsis grubunda daha çok intraepitelyal nötrofilik infiltrasyon, plazma hücreleri ve ödem görülürken, infiltrasyon ve hiperemi kefir grubunda azdı. Sonuç olarak, kefir deneysel sepsiste organ disfonksiyonunu azaltmıştır. Sepsisle, şokla yada lokal sistemik inflamasyonla ilişkili organ fonksiyon bozukluğunda kefirin yararlı bir tedavi olabileceğini düşünmekteyiz. Kefir has antioxidant, antitumor, and anti-inflammatory properties. However, it remains unknown whether kefir has any protective effects on sepsis. The purpose of this study was to demonstrate whether kefir prevents organ dysfunction in animals with sepsis. Rats were randomized into four groups. The control group (group I, n=7) did not receive any treatment. The kefir group (group II, n=7), only received 2 ml kefir. E. coli was injected to the remaining groups intraperitoneally after general anesthesia. Five hours after injection, 7 rats received placebo (group III), and 7 rats received kefir (group IV) for seven days. All rats were killed on postsepsis day eight and a midline laparotomy was performed. Lungs, livers, kidneys, spleen and large bowels were excised for evaluation of the degree of inflammation and tissue alterations histopathologically. Lungs; edema and congestion of sepsis and kefir group was less the sepsis group.In liver; widespread hydrophic degeneration of hepatocytes were seen in sepsis group. There was no hydrophic degeneration of hepatocytes and no portal inflammation in the sepsis and kefir group. In kidneys; sepsis group showed severe acute tubular necrosis which was more restricted in sepsis and kefir group than the sepsis group. Spleen; expansion in red pulp, bleeding, inflamed cell infiltration of mixed type in focal areas of sepsis and kefir group was less the sepsis group. In large bowel; sepsis group showed edema and prominent intraepithelial infiltration of neutrophil leucocytes and plasma cells. Inflammation and hyperemia in the lamina propria of the sepsis and kefir group were less than the sepsis group. Kefir reduced organ dysfunction in rats with experimentally formed sepsis. We propose that kefir may be useful in the therapy of organ dysfunction due to sepsis, shock, and other diseases associated with local or systemic inflammation.
