Özet
Bu çalışmanın amacı; önlenebilir kronik hastalıkların giderek artan yükünü hafifletmek için birinci basmakta beslenme ile ilgili müdahaleler hakkında önleyici danışmanlık önündeki engellerin belirlenmesi, koruyucu sağlık hizmetleri kapsamında multidisipliner ekip içerisinde diyetisyenin tanınması ve öneminin farkındalığını sağlamaktır. Çalışmamız 1 Şubat 2022 – Mayıs 2022 tarihleri arasında Trakya Üniversitesi Tıp Fakültesi Aile Hekimliği Anabilim Dalı’ında görev alan ve çalışmaya katılmayı kabul eden 93 asistan hekim ile yüz yüze görüşme tekniği kullanılarak yapılmıştır. Kesitsel ve tanımlayıcı olarak planlanan bu çalışmada veriler; araştırmacılar tarafından hazırlanan, katılımcıların sosyodemografik bilgilerine yönelik sorular ve beslenme, diyetisyen ve ekip çalışması konusunda bilgi, tutum ve davranışlarını tespit etmeyi amaçlayan sorulardan oluşan anket uygulanarak toplanmıştır. Çalışmaya katılan asistan hekimlerin sadece 3’ü (%3,23) mesleki eğitimleri esnasında beslenme eğitimi aldığını ifade ettimiştir. Hekimlerin 35’i (%37,63) daha önce bir diyetisyen ile birlikte çalıştığını belirtmiştir. Katılımcıların sağlık ekibinde yer alacak diyetisyende olmasını istedikleri en önemli özellik 83 (89,35) kişi ile eğitimi yanıtı olmuştur. Medyada yer alıyor olması hiçbir katılımcı tarafından öncelikli tercih nedeni olarak belirtilmemiştir. Daha önce birinci basamak sağlık hizmetinde görev alan asistan hekimlerin beslenme bilgi sorularına verdikleri doğru yanıt sayıları daha önce birinci basmak sağlık hizmeti deneyimi olmayan asistan hekimlere göre anlamlı olarak daha yüksek bulunmuştur (p<0,001). Literatür tarandığında birinci basmak sağlık hizmetlerinde beslenme danışmanlığının diyetisyen tarafından verilmesinin önemli olduğunu ve hasta davranışını değiştirmede etkili olabileceğini gösteren çalışmalar vardır. Bu çalışma gelecekte çoğunlukla birinci basmak sağlık hizmetlerinin yürütülmesinde görev alacak olan asistan hekimlerin bu noktada ileride diyetisyen ile birlikte çalışmaya hevesli olduğu görülmektedir. Birinci basmak sağlık hizmetlerinin yürütülmesinde multidisipliner bir ekibin varlığı ve ekip içerisinde diyetisyenin varlığı beslenme ile alakalı konularda oldukça önemlidir. Bulaşıcı olmayan hastalıkların önlenmesinde güçlendirilmiş ve donanımlı bir ekip çalışması birinci basamak sağlık hizmetlerinin sağlıklı yürütülmesi için en doğru yaklaşım olacaktır.
he aim of our study was to determine the barriers to preventive counseling about nutrition related interventions in primary care to alleviate the increasing burden of preventable chronic diseases, and to ensure the recognition and awareness of the importance of dietitians within the multidisciplinary team within the scope of preventive health services. Our study was conducted between February 1, 2022 and May 1, 2022 using face-toface interview technique with 93 resident physicians working in Trakya University Faculty of Medicine, Department of Family Medicine and who agreed to participate in the study. In this cross-sectional and descriptive study, data were collected by applying a questionnaire prepared by the researchers, consisting of questions about the sociodemographic information of the participants and questions aiming to determine their knowledge, attitudes and behaviors about nutrition, dietitian and teamwork. Only 3 (3.23%) of the resident physicians who participated in the study stated that they received nutrition education during their professional training. Thirty-five of the physicians (37.63%) stated that they had worked with a dietitian before. The most important feature that the participants wanted the dietitian to have in the health care team was education with 83 (89.35%) respondents. Being featured in the media was not stated as the primary reason for preference by any of the participants. The number of correct answers given to the nutrition information questions by the resident physicians who had previously worked in primary health care was found to be significantly higher than the residents who had no previous primary health care experience (p<0.001). When the literature is reviewed, there are studies showing that nutrition counseling by a dietitian in primary health care is important and may be effective in changing patient behavior. In our study, it was observed that physician assistants, who will mostly take part in primary health care services in the future, were eager to work with dietitians in the future. The presence of a multidisciplinary team in primary health care services and the presence of a dietitian within the team is very important in nutrition-related issues. An empowered and well-equipped teamwork in the prevention of non-communicable diseases will be the best approach for the healthy execution of primary health care services.
