Akut unilateral üretreral obstrüksiyonda Rosuvastatin ve l-Karnitinin böbrek fonksiyonu ve histopatolojisi üzerine etkileri

Özet
Üriner obstrüksiyon tüm yaş gruplarında oldukça sık karşılaşılan bir durumdur. Üriner obstrüksiyonda böbrekte reaktif oksijen ürünlerinin arttığı ve sonuçta bu reaktif oksijen ürünlerinin apoptozisi indüklediği düşünülmektedir. Tek taraflı üreteral obstrüksiyonda rosuvastatin ve L-karnitinin oluşacak böbrek hasarı üzerine olan etkilerini incelemeyi amaçladık. Çalışmamızda Trakya Üniversitesi Deney Hayvanları Üretim ve Araştırma Laboratuvarı'nda yetiştirilen, ağırlıkları 200 ile 250 gr arasında değişen 4 aylık 60 adet dişi, Wistar Albino türü sıçan kullanıldı. Sıçanlar 6 gruba ayrıldı. Rosuvastatin ve tek taraflı üreteral obstrüksiyon + rosuvastatin gruplarına 6 gün 20 mg/kg intraperitoneal rosuvastatin, L-karnitin ve tek taraflı üreteral obstrüksiyon + L-karnitin gruplarına 6 gün 300 mg/kg intraperitoneal L-Karnitin uygulandı. Serum fizyolojik ve tek taraflı üreteral obstrüksiyon gruplarına 6 gün intraperitoneal serum fizyolojik uygulaması yapıldı. Bu ilaç uygulamaları devam ederken dördüncü gün grupların ilaç uygulamalarını takiben tek taraflı üreteral obstrüksiyon + rosuvastatin, tek taraflı üreteral obstrüksiyon + L-karnitin ve tek taraflı üreteral obstrüksiyon gruplarına tek taraflı üreteral obstrüksiyon yapıldı. SHAM, rosuvastatin ve L-karnitin gruplarına tek taraflı üreteral obstrüksiyon yapılmadan sadece laparatomi yapıldı. Yedinci gün sıçanlar anestezi altındayken radyonükleid glomerüler filtrasyon hızı çalışıldı. Ardından sıçanların obstrükte edilen böbrekleri alınıp sakrifiye edildi. Böbrek dokusunda malondialdehit, katalaz, süperoksit dismutaz, glutatyon, glutatyon peroksidaz, total antioksidan kapasite düzeyleri ile böbrek dokusunda histopatolojik değişiklikler incelendi. 45 Tek taraflı üreteral obstrüksiyonun 3. gününde glomerüler filtrasyon hızının etkilenmediği görüldü. Rosuvastatinin oluşturulan tek taraflı üreteral obstrüksiyonda malondialdehit, glomerüler skleroz ve intertisyel fibrozisi anlamlı olarak azalttığı görüldü. L- karnitinin oluşturulan tek taraflı üreteral obstrüksiyonda malondialdehit, tübüler hasar skoru, glomerüler skleroz ve intertisyel fibrozisi anlamlı olarak azalttığı görüldü. Rosuvastatin ve L- karnitin malondialdehit, glomerüler skleroz ve intertisyel fibrozisi benzer oranda azalttığı ve gruplar arasında anlamlı fark olmadığı görüldü. Bu sonuçlarla tek taraflı üreteral obstrüksiyonda L-karnitin ve rosuvastatin böbrek hasarını azaltmada olumlu etkileri olduğunu düşünüyoruz. Abstract
EFFECTS OF ROSUVASTATIN AND L-CARNITINE ADMINISTRATION ON RENAL FUNCTION AND HISTOPATHOLOGY AT ACUT UNILATERAL URETERAL OBSTRUCTION SUMMARY Urinary obstruction is common in all age groups. In urinary obstruction, increased reactive oxygen species in kidney and induced apoptosis with reactive oxygen species is thought. We aimed at to study the effects on the kidney, when we gived rosuvastatin and L- carnitine at unilateral ureteral obstruction. In our study, 200-250 grams in weight, 60 female Wistar Albino rats, were used. The rats were randomly divided into 6 groups. Rosuvastatin 20 mg/kg intraperitoneally administered to rosuvastatin and unilateral ureteral obstruction + rosuvastatin groups for 6 days. L-carnitine 300 mg/kg intraperitoneally administered to L-carnitine ve unilateral ureteral obstruction + L-carnitine for 6 days. Saline was applied intraperitoneally to SHAM and unilateral ureteral obstruction groups for 6 days. Pharmaceutical applications of the groups following the fourth day, unilateral ureteral obstruction was applied to unilateral ureteral obstruction + rosuvastatin, unilateral ureteral obstruction + L-carnitine and unilateral ureteral obstruction groups after drug applications. Only laparotomy was performed to SHAM, rosuvastatin ve L-carnitine groups. Radionuclide glomerular filtration rate were studied in rats under anesthesia, the seventh day. Then the obstructed kidneys of the rats were taken and sacrificed. On renal tissue malondialdehit, catalase, super oxide dismutase, glutathione, 47 glutathione peroxidase, total antioxidant capacity levels and histopathological changes were examined. Glomerular fitration rate was not affected by unilateral ureteral obstruction at 3rd day. Rosuvastatin administration to unilateral ureteral obstruction, signifacantly reduced malondialdehit, glomerular sclerosis and interstitial fibrosis were examined. L-carnitine administration to unilateral ureteral obstruction, signifacantly reduced malondialdehit, tubular damage score, glomerular sclerosis and interstitial fibrosis were examined. Rosuvastatin and L-carnitine decrease similar percentage of malondialdehit, glomerular sclerosis and interstitial fibrosis. We think that administration of rosuvastatin and L-carnitine at unilateral ureteral obstruction has positive effects in reducing kidney damage.
