Özet
Metabolik sendrom, yaşam kalitesini bozup birey ve dolayısı ile toplum sağlığı için önemli bir risk oluşturan, tüm doku ve sistemleri ilgilendiren ve olumsuz etkileyen bir tablo olup santral obezite, dislipidemi, hiperglisemi ve yüksek kan basıncını içeren bir hastalık kümesini kapsar. Araştırmadaki öncelikli amacımız; kan damarı ve akımı hakkında bilgi edinmemizi sağlayan, non-invaziv, ucuz, tekrarlanabilir bir teknik olan renkli dopler ultrasonografi ile metabolik sendromlu hastaların orbita kan akım parametrelerini değerlendirmek ve kontrol grubu ile karşılaştırmaktı.Çalışmaya endokrinoloji polikliniğine başvuran metabolik sendrom tanısı almış 40 hasta ve kontrol grubunu oluşturan 30 sağlıklı birey alındı. Olguların, renkli dopler ultrasonografi ile elde edilen orbita kan akım parametreleri, karotis intima-media kalınlığı ve subkutan yağlı doku kalınlığı ile demografik verileri kaydedildi. Metabolik sendromun komponentleri ayrı ayrı ve beraber ele alınarak orbita kan akımı üzerine etkileri araştırıldı. Ayrıca olgularda karotis intima-media kalınlığı ile abdominal subkutan yağlı doku kalınlığının da orbita kan akımı üzerine etkileri araştırıldı.Sonuçta, metabolik sendromun her üç arterde (oftalmik arter, santral retinal arter ve posterior silier arter) akım hızlarını ve pulsatilite indeks ile rezistiv indeks değerlerini etkilediğinden söz etmek mümkündü. Metabolik sendroma yüksek orbital akım hızları, pulsatilite indeks, rezistiv indeks ve intima media-kalınlığında artış eşlik etmekteydi.
The metabolic syndrome consits of a cluster of diseases, including obesity, dyslipidemia, hyperglisemia and high blood pressure, that concerns all tissue and organ systems and adversely affect the quality of life and being so also an important risk factor for the public health.The primary goal of our study is to invastigate the orbital blood flow parameters in patients with metabolic syndrome and to compare the results with the cotrol group using color dopler ultrasonography which is non-invasive, cheap and reproducible way of evaluating the blood vessels and blood flow.Fourty patients with metabolic sydrome who had been followed in the endocrinology clinic of our hospital and 30 healthy volunteer subjects were included in our study. Orbital blood flow parameters obtained by color doppler ultrasonography, intima-media thickness of carotid artery, the thickness of subcutaneous fat tissue and demographic information was recorded. The effect of the each of the component of the metabolic sydrome alone and in combination to the orbital blood flow were investigated. In addition, the relation the carotid intima-media thickness and abdominal subcutaneous fat tissue thickness with the orbital blood flow were investigated.In conclusion, it is possible to mension that metabolic syndrome effects on the velocity, pulsatility index and resistive index values of each of these three arteries (ophthalmic artery, central retinal artery and posterior ciliary artery). Our study showed that metabolic syndome accompanied by high orbital blood flow velocities with increased pulsalite index and resistive index values and increased carotid intima-media thickness.
