İleri derecede prematüre yenidoğanlarda ilk 5 gün içerisinde alınan tam kan sayımı indekslerinin morbidite ve mortalite tahmininde bulunup bulunamayacağını değerlendirmesini amaçladığımız bu çalışmada olgular Trakya Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim dalı Neonatoloji kliniği tarafından Ocak 2015 – Ocak 2020 tarihleri arasında tarafımızca takip edilen 150 bebek dahil edildi. RDS, intraventriküler hemoraji, bronkopulmoner displazi (BPD), nekrotizan enterokolit (NEK), prematüre retinopatisi (PR), patent duktus arteriosus (PDA), PVL morbiditelerin varlığı kaydedildi. Hastaların postnatal 1.gün, 3.gün, 5.gününe ait hematolojik indeksleri kaydedildi. Ortalama gestasyonel yaşın 28,74±2,26 hafta olarak belirlendiği çalışmada olguların morbiditeleri değerlendirildiğinde; %12,07 oranında orta ve ağır BPD, %37,93 evre 2 ?NEK , %12,9 hemodinamik olarak anlamlı PDA, %12,9 tedavi gerektiren PR ve %6,89’ünde evre 2 ? İVK rapor edilmiştir. Verilerimiz, yaşamın ilk 5 günündeki hematolojik parametreler ile prematüre bebeklerde görülen PDA, BPD, ROP, İVK, NEK ve sepsis gibi morbiditeler arasındaki ilişki hakkında bilgi azlığına daha fazla kanıt sunmaktadır. Bakılan hematolojik indekslerden öngörü düzeyi en yüksek hematolojik parametreler Hgb, Hct ve KKS olarak saptandı. Son yüzyılda artmış inflamatuvar yanıtın göstergesi olarak hematolojik parametreler özellikle prematüre bebeklerin mortalite ve morbiditesinde incelenmiştir. Konuyla alakalı daha çok araştırma yapılması gerekmektedir.
In this study we aimed to evaluate if the complete blood count indexes of very extremely preterm newborns in the first 5 days help the prediction of mortality and morbidity, 150 babies that were being followed in Newborn Intensive Care Unit of Trakya University Pediatric Health and Diseases Department between January 2015 and January 2020 were included. The presence of RDS, Intraventricular hemorrhage, necrotizing enterocolitis (NEC), Patent ductus arteriosus (PDA), bronchopulmonary dysplasia (BPD), retinopathy of prematurity (PR), PVL were recorded. Patients' postnatal first day, third day and fifth day hematological indexes were recorded. İn this study that the mean gestational age was determined as 28,74±2,26, when the morbidity was evaluated ; 12,07% medium and severe BPD, 37,93% phase 2 >=NEK, 12.9% hemodynamically important PDA, 12.9% PR that requires treatment and 6.89% phase 2 >=IVH were reported. Our data offers more evidence for the lack of information about the relations between hematological parameters in the first 5 days of life and morbidities like PDA, BPD, ROP, IVH, NEK and sepsis in preterm newborns. Hgb, Hct and KKS were recorded to have the highest levels of forethought between studied hematological indexes. Especially in the last century, hematological parameters have been studied as indicators of increased inflammatory response in mortality and morbidity in preterm newborns. But there is a need for more studies to be prosecuted in this matter.
