Uyku deprivasyonunun vizüel reaktivite üzerine olan etkisinin transkraniyal doppler ile değerlendirilmesi

Özet
yku deprivasyonun en sık sebepleri modern yaşam koşulları ve iş ile ilgili faktörlerdir.Bundan dolayı bu durum çok sayıda insanı etkiler. Yetersiz uyku ile kişilerde dikkat azalması ve kognitif fonksiyonlarda bozulma görülür.Tüm bu nedenlerle bu çalışmada, uyku deprivasyonunun vizüel reaktivite üzerine olan etkisinin ve vizüel uyarı sonrasında her iki arka serebral arterde meydana gelen kan akım hızı değişimlerinin transkraniyal doppler ile değerlendirilmesi amaçlanmıştır. Bu çalışmaya, Trakya Üniversitesi Tıp Fakültesi Hastanesinde nöbet tutan ve kontrol grubu kendisi olan toplam 20 sağlık personeli alındı. Çalışmaya alınan kişiler 20-40 yaş arası sağlıklı, genç, erişkin erkeklerdi. Katılımcılara, yirmidört saatlik uykusuzluk sonrası ve dinlenmiş olarak geldikleri mesai gününde transkraniyal doppler ile vizüel reaktivite testi yapıldı. Her iki arka serebral arter aynı anda monitorize edildi. Vizüel reaktivitedeki maksimum değerler ile, istirahat sonrası minimum değerler kaydedildi. Reaktiviteler hesaplandı. Daha sonra uyku deprivasyonu ve istirahat sonrası elde edilen vizüel reaktivite değişimleri kıyaslandı. Yapılan değerlendirmeler sonrası, sağ serebral hemisferin sol hemisfere göre uyku deprivasyonundan daha fazla etkilendiği saptandı. Sağ arka serebral arterden istirahat sonrası elde edilen kan akım hızı değerleri, uyku deprivasyonu sonrası elde edilen değerlere göre anlamlı derecede yüksek bulundu (p<0.001). Hem istirahat hem de uyku deprivasyonunda vizüel reaktivite ile akım hızlarının anlamlı derecede arttığı görüldü (p<0.001). Uyku deprivasyonu sonrası vizüel reaktivite değerlerinin, istirahat sonrası vizüel reaktivite değerlerine göre anlamlı derecede düşük olduğu görüldü (p<0.001) Sonuç olarak, çalışmamız kolay uygulanabilir ve noninvazif bir teknik olan transkraniyal dopplerin uyku deprivasyonunun etkilerini incelemek ve bazal serebral arterlerin kan akım hızını ölçmek için iyi bir metod olduğunu olduğunu göstermiştir.
Abstract
The most common causes of sleep deprivation are related to lifestyle and occupational factors which affects a considerable number of people. Without adequate sleep, humans show reduced alertness and impairments in cognitive functions. Therefore, we aimed to evaluate the effect of sleep deprivation on visual reactivity and blood flow velocity changes after visual stimuli on both posterior cerebral arteries with transcranial doppler ultrasonography. In this study, 20 healthcare personals who were in duty in Trakya University Medical Hospital were included and besides, they were participated as a control group either. Participants were 20-40 aged, healthy, young men. Visual reactivity test was applied to participants by trancranial doppler ultrasonography after 24 hours deprivation and on rested days after sleeping within the working hours. Both posterior cerebral arteries were monitorized at the same time. The maximum values on visual reactivity and minimum values on resting were recorded. Reactivity was calculated. Subsequently, acquired visual reactivity changes after sleep deprivation and resting were compared. Following assessments, it was found that right cerebral hemisphere was more adversely affected from sleep deprivation than left serebral hemisphere. Blood flow velocity values obtained from right posterior cerebral artery following resting were found to be significantly higher than the values obtained following sleep deprivation (p<0.001). Both following resting and sleep deprivation, blood flow velocities following visual reactivity was found to be increased significantly (p<0.001). Visual reactivity values following sleep deprivation were found to be significantly lower than the visual reactivity values following resting (p<0.001). In conclusion, our study showed that transcranial doppler is a very good method which is easily applicable and noninvasive for evaluation the effects of sleep deprivation and measuring the blood flow velocities of the basal cerebral arteries.
