Özet
Bu çalışmanın amacı şiddetli preeklampside plasental apopitozun otoantikor ve proinflamatuar sitokinlerle araştırılmasıdır. Trakya Üniversitesi Tıp Fakültesi Sağlık Araştırma ve Uygulama Hastanesi Kadın Hastalıkları ve Doğum Bölümü’nde 01.11.2012-01.05.2015 tarihleri arasında doğumu gerçekleşen ağır preeklamptik, HELLP sendromlu ve sağlıklı toplam 66 gebe çalışmaya dahil edildi. Olguların plasenta ve umbilikal kord kesitlerinden Trakya Üniversitesi Tıp Fakültesi Sağlık Araştırma ve Uygulama Hastanesi Patoloji Bölümü’nde parafin blokları oluşturulup otoantikor ve proinflamatuar sitokinlerin( AT1-AA, sFlt-1, TNF-?, IL-6 ve IL-10) immunreaktivite skoru değerlendirildi.Ağır preeklamptik gebelerin, HELLP Sendromlu ve kontrol grubu gebelerin plasental villuslarının arter ve venlerinde AT1-AA’nın daha yüksek olduğu bulundu. HELLP Sendromu, ağır preeklamptik ve kontrol grubunun plasental villusları arasında sFlt-1 düzeyinde anlamlı fark olmadığı bulundu. HELLP Sendromlu, ağır preeklamptik ve kontrol grubunun plasental villuslarında IL-6,IL-10 ve TNF-? düzeyleri arasında anlamlı fark saptanmadı. HELLP Sendromu, ağır preeklampsi ve kontrol grubunun umbilikal kordlarındaki AT1-AA, sFlt-1, TNF-?, IL-10 ve IL-6 düzeyleri arasında anlamlı fark bulunmadı. Ağır preeklamptik ve HELLP Sendromlu gebelerin gestasyonel haftalarının AT1-AA, sFlt-1, TNF-?, IL-6 ve IL-10 düzeyleri üzerine bir etkisi olmadığını saptadık.Sonuç olarak ağır preeklamptik, HELLP Sendromlu ve kontrol grubu gebelerin plasentaları otoantikorlar ve proinflamatuvar sitokinlerle araştırıldı. Ağır preeklampsi ve AT1-AA düzeyleri arasında anlamlı bir ilişki olduğu ama ağır preeklamptik, HELLP Sendromlu ve kontrol olguları arasında sFlt-1, IL-6, IL10 ve TNF-? düzeyleri açısından anlamlı bir ilişki olmadığı saptandı. Ayrıca Ağır preeklamptik ve HELLP sendromlu gebelerin gestasyonel haftalarının AT1-AA, sFlt-1, TNF-?, IL-6 ve IL-10’a bir etkisi olmadığını saptadık.

The aim of this study is to search the severe preeclampsia with the autoantibody and the proinflammatory cytokines of the placental apoptosis. 66 pregnant women who had given birth between the dates of 01.11.2012-01.05.2015 with either severe preeclampsia, HELLP syndrome or with no medical problems (totally healthy) in the Trakya University Medicine Faculty Health Research and Application Hospital were included to the study. Paraffin blocks were created from the placental sections of the each case in the Pathology Department of the Trakya University Medicine Faculty Health Research and Application Hospital and ımmunreactivity score of the autoantibody and the proinflammatory cytokines (AT1-AA, sFlt-1, TNF-, IL-6 and IL-10) were evaluated (valued).Higher levels of AT1-AA in the artery and veins of the placental villies were determined in the placentas of the pregnant women with severe preeclampsia according to the ones with HELLP Syndrome and control groups. No significant differences were found (determined) in the sFlt-1 levels of the placental villies belonged to the pregnants with HELLP syndrome, severe preeclampsia and control group. The placentas of the pregnants who had HELLP syndrome and severe preeclampsia were compared with the control group, there were no significant differences in the levels of IL-6, IL-10, and TNF of the placental villies. The pregnants who had HELLP syndrome and severe preeclampsia were compared with the control group, there were no significant differences in the levels of AT1-AA, sFlt-1, IL-6, IL-10, and TNF of the umbilical cord. We determined that gestational weeks had no effect on the AT1-AA, sFlt-1, TNF-, IL-6 and IL-10 levels of the pregnants having HELLP syndrome and severe preeclampsia.As a result placentas of the pregnants having HELLP syndrome, severe preeclampsia and control group were searched with autoantibodies and the proinflammatory cytokines. A significant relationship was determined between severe preeklampsia and AT1-AA ımmunreactivity score, while no significant relationship for the the levels of sFlt-1, IL-6, IL-10, and TNF ımmunreactivity was determined between HELLP syndrome, severe preeclampsia and control group. Nonetheless we identifed that the gestastional weeks of the pregnants with HELLP syndrome and severe preeclampsia had no effect on the AT1-AA, sFlt-1, IL-6, IL-10, and TNF ımmunreactivity scores.
