Psoriasiste hastalık şiddeti ve hastalık algısı arasındaki ilişkinin değerlendirilmesi

Özet
Psoriasis; kozmetik kaygılara, kaşıntı gibi yakınmalara yol açmakta ve hastaların yaşamlarını olumsuz yönde etkileyebilmektedir. Bu nedenle psoriasislilerde depresyon ve anksiyete bozukluğu, organik hastalıkların hemen hepsinden fazla bulunmuştur. Çalışmamızda Trakya Üniversitesi Tıp Fakültesi Deri ve Zührevi Hastalıklar Polikliniği'ne başvuran psoriasisli hastaların hastalık algısı, anksiyete ve depresyon düzeylerinin tespit edilmesi ve bunların psoriasis şiddeti ile ilişkisinin gösterilmesi amaçlandı. Çalışmaya, tanıları klinik olarak konulan ve yaşları 18-67 arasında değişen 47'si erkek, 39'u kadın 86 psoriasisli hasta dâhil edildi. Hastalara sosyodemografik anket, kısa hastalık algı ölçeği, hastane anksiyete ve depresyon ölçeğinden oluşan 37 soruluk bir anket uygulandı. Hastalık şiddeti, psoriasis alan ve şiddet indeksi ile değerlendirildi Anket sonuçlarına göre, hastalık şiddeti, erkeklerde kadınlardan daha fazlaydı. Saçlı deri tutulumu olan psoriasislilerde, hastalık daha şiddetliydi. Hastalık şiddeti ile hastalık algı düzeyi ve anksiyete düzeyi arasında korelasyon yoktu. Hastalık süresi ile hastalık şiddeti arasında korelasyon saptandı. Hastalık algısı ile anksiyete düzeyi ve depresyon düzeyi arasında korelasyon olduğu belirlendi. Hastalık şiddeti ile depresyon düzeyi arasında pozitif yönde ilişki tespit edildi. Sonuç olarak; psoriasiste tedavi, yaşam kalitesi ve hastalık süresinin, anksiyete ve depresyon gibi psikiyatrik problemlerden etkilenebileceği düşüncesindeyiz. Psoriasiste sadece deri lezyonları ele alınmamalı; hastalık algısı, anksiyete bozukluğu, depresyon ve eşlik edebilecek diğer psikiyatrik durumlar, psikiyatrik destek gerekliliği açısından dikkatle değerlendirmelidir.
Psoriasis causes cosmetic concerns and complaints like itchiness and it may adversely affect the lives of patients. Therefore, among the psoriasis patients, depression and anxiety disorder have been found more than almost all of organic diseases. In our study, it was aimed to determine the illness perception, anxiety and depression levels and to demonstrate the correlation of these levels with psoriasis severity among the psoriasis patients who admitted to Dermatology Department of Trakya University Medical Faculty. The study includes 86 clinically diagnosed psoriasis patients that 47 of them are male and ranging in age from 18 to 67. Questionnaires that consist of 37 questions from Brief Illness Perception Questionnaire, Hospital Anxiety and Depression Scale and sociodemographic questionnaire were applied to the patients and the disease severity was evaluated with Psoriasis Area Severity Index. As a result of the questionnaires, disease severity was higher in males than females. In patients with scalp involvement, disease was more severe. There was no correlation between disease severity and illness perception or anxiety levels. We found that, duration and severity of disease were correlated. It was found that illness perception affects anxiety and depression levels in patients with psoriasis. Depression levels were higher in patients with severe psoriasis. As a conclusion, we are of the opinion that treatment, recovery time and quality of life can be affected by psychiatric problems of patient such as anxiety and depression in psoriasis patients. As handling the chronic disease psoriasis, not only the skin lesions should be considered but also the illness perception, anxiety disorder, depression and related possible psychiatric conditions should be evaluated by clinicians in terms of necessity of psychiatric support.
