Öğe Arterial distensibility in Wegener's granulomatosis(Turkish Soc Cardiology, 2007) Yildiz, Mustafa; Soy, Mehmet; Kueruem, Turhan; Yildiz, Banu SahinObjective: The purpose of this study was to test the hypothesis; that chronic inflammation may impair vascular function and lead to an increase of arterial pulse wave velocity (PWV) in patients with Wegener's granulomatosis (WG). Methods: We recruited 5 patients with WG and 5 healthy age and sex matched controls in this cross-sectional case-controlled study. Aortic PWV was determined by using an automatic device (Complior Colson, France), which allowed on-line pulse wave recording and automatic calculation of PWV. Results: The carotid-femoral (aortic) PWV was increased in patients with WG as compared with control group (p=0.04). Although we found positive correlation between MAN and heart rate (r=0.75, p=0.01), we did not find any significant correlation between PWV and anthropometric and other hemodynamic parameters (p>0.05). In addition, we found positive correlation between PWV and erythrocyte sedimentation rate in patients with WG (r=0.90, p=0.03). Conclusion: Pulse wave velocity is increased and arterial distensibility decreased in patients with WG. Measurements of carotid-femoral (aortic) PWV may provide an easy and noninvasive technique to identify patients at increased risk of arterial disease.Öğe Arterial distensibility in Wegener’s granulomatosis: a carotid - femoral pulse wave velocity study(2007) Yıldız, Mustafa; Kürüm, Turhan; Soy, Mehmet; Yıldız, Banu ŞahinAmaç: Çalışmanın amacı, kronik enflamasyon Wegener granülomatoz (WG)’lu hastalarda vasküler fonksiyonları bozup arteryel nabız dalga hızı (NDH)’nı artırabilir, hipotezini araştırmaktır.Yöntemler: Yaş ve cinsiyet olarak uyumlu 5 WG’lu hasta ve 5 sağlıklı kontrol grubu çalışmaya dahil edildi. Aortik NDH, on-line nabız dalga kaydına izin veren ve otomatik ölçüm yapan Complior Colson (Fransa) cihazı ile belirlendi.Bulgular: Karotis-femoral (aortik) NDH kontrol grubuna göre WG’lu hastalarda daha yüksek bulundu (p=0.04). Nabız dalga hızı ve kalp hızı arasında pozitif yönde korelasyon saptarken (r=0.75, p=0.01), NDH ile antropometrik ve diğer hemodinamik değişkenler arasında anlamlı korelasyon izlemedik (p>0.05). Ek olarak WG’lu hastalarda, NDH ve sedimantasyon arasında pozitif korelasyon saptadık (r=0.90, p=0.03). Sonuç: Wegener granülomatoz’lu hastalarda NDH artmış arteryel genişleyebilirlik azalmıştır. Karotis-femoral (aortik) NDH artmış arteryel hastalık riski olan hastaların teşhisinde kolay ve non-invazif bir teknik olabilir. (Anadolu Kardiyol Derg 2007; 7: 281-5)Öğe Churg-Strauss syndrome associated with montelukast(Turkish Assoc Tuberculosis & Thorax, 2008) Ozlen, Burcu; Ozdemir, Levent; Eskitutuncu, Banu; Cevirme, Leyla; Kurtar, Nazan; Soy, Mehmet; Hatipoglu, Osman NuriA 47 year old woman who had a history of asthma for 15 years referred to our hospital because of infiltrates on her chest radiograph that not responded to antibiotic treatment. We found that she had eosinophilia in peripheral blood (38%) and bronchoalveolar lavage (54%), nasal polyposis, and transient pulmonary infiltrates, and in the base of these findings we diagnosed as Churg-Strauss Syndrome (CSS). She has been using montelukast for 2 years. By examining her previous medical records, we observed that while eosinophil rates in peripheral blood were normal before montelukast usage, after this therapy eosinophil rates were greater 10 percent. Therefore, we thought that CSS was to be associated with montelukast usage. After just montelukast therapy was discontinued, clinical and radyographic parameters and the eosinophil counts (20%) improved. We present this case of CSS associated with montelukast in whom spontaneous remission was observed without using corticosteroids and cytotoxic agents.Öğe Cutaneous findings in patients with primary Sjogren's syndrome(Springer, 2007) Soy, Mehmet; Piskin, SueleymanSjogren's syndrome (SS) is an autoimmune disease that is characterized by exocrine gland involvement. It is reported that the skin is affected in nearly half of SS patients. Cutaneous manifestations consist of xerosis, angular cheilitis, eyelid dermatitis, pruritus, cutaneous vasculitis (frequently manifesting as palpable purpura), and erythema annulare. Most of them are nonspecific and less severe than the oral, ocular, or musculoskeletal symptoms. We present two patients with primary SS who have many cutaneous manifestations including erythema annulare and pernio-like lesions and review the literature about this topic.Öğe Frequency of rheumatic diseases in patients with autoimmune thyroid disease(Springer, 2007) Soy, Mehmet; Guldiken, Sibel; Arikan, Ender; Altun, Betul Ugur; Tugrul, ArmaganWe aimed to investigate the frequency of rheumatic diseases in patients suffering from autoimmune thyroid diseases (ATD). Sixty-five patients (56 F, 9 M), who were followed by diagnosis of ATD, were questioned and examined for the presence of rheumatic disease. Basic laboratory tests and antithyroid antibodies, antinuclear antibody and rheumatoid factor (RF) levels were also measured by appropriate methods. Various rheumatic diseases were detected in 40 (62%) of patients with ATD. The most frequent rheumatic conditions were fibromyalgia, recurrent aphthous stomatitis, osteoarthritis, keratoconjunctivitis sicca and xerostomia and carpal tunnel syndrome which were detected in 20 (31%), 13 (20%), 10 (15%), 9 (14%) and 8 (12%) of patients, respectively. Autoimmune diseases, except Sjogren's syndrome, which were detected in ten patients with ATD, are as follows-vitiligo: two; autoimmune hepatitis: two; oral lichen planus: one, ulcerative colitis: one, inflammatory arthritis in four patients (two of them had rheumatoid arthritis, one had psoriasis and psoriatic arthritis and one had mixed collagen tissue disease). RF was positive in two patients, one of them had rheumatoid arthritis and FANA was positive in six (9%) patients; all of them had hypothyroidism. The frequency of rheumatic diseases seems to be higher in patients suffering from ATD. Initial evaluation and a regular checking for rheumatic diseases in patients suffering from ATD were recommended.Öğe Joint and nail involvement in Turkish patients with psoriatic arthritis(Springer Heidelberg, 2008) Soy, Mehmet; Karaca, Nuran; Umit, Elif Ufur; Bes, Cemal; Piskin, SueleymanPsoriatic arthritis is an autoimmune, chronic, systemic inflammatory disorder characterized by the association of arthritis with psoriasis. In this paper, we explore the characteristics of joint and nail involvement in Turkish patients with psoriatic arthritis. Forty patients with psoriasis (M/F, 18/22) and 49 (M/F, 25/24) subjects with psoriatic arthritis were included in the study. Clinical characteristics of the patients were recorded. The distribution of the subjects with arthritis: (according to the clinical and radiological findings): polyarticular, 65%; oligoarticular, 23%; isolated axial involvement, 7.7%; arthritis mutilans, 3.8%; sacroiliitis, 19%. Nail involvement was significantly higher among patients with arthritis; i.e., 91 versus 32%; (P < 0.05). There were no correlation between the skin involvement pattern and the arthritis type (P > 0.05). Nevertheless, no relation was observed between the psoriasis duration and arthritis (P > 0.05). Nail involvement is a frequent feature of the psoriatic arthritis which may be a useful finding for differential diagnosis of psoriatic arthritis from other inflammatory arthropathies.Öğe Kikuchi-Fujimoto disease coexisted with Sjogren's syndrome(Springer, 2007) Soy, Mehmet; Peynirci, Hande; Bilgi, Selcuk; Adali, Mustafa Kemal; Guresci, ServetHere we described a case of primary Sjogren's syndrome that coexisted with Kikuchi-Fujimoto disease.Öğe Montelukast ile ilişkili Churg Strauss sendromu(2008) Özlen, Burcu; Çevirme, Leyla; Eskitütüncü, Banu; Soy, Mehmet; Hatipoğlu, Osman Nuri; Kurtar, Nazan; Özdemir, LeventOn beş yıldır astım öyküsü olan 47 yaşında bir kadın hasta antibiyotik tedavisine yanıt vermeyen radyolojik infiltrasyonu nedeniyle hastanemize sevkedildi. Hasta 2 yıldır montelukast kullanıyordu. Periferik kanda eozinofili (%38), bronkoalveoler lavajda eozinofili (%54), nazal polip, akciğer grafisinde geçici infiltrasyonlar saptanması üzerine Churg-Strauss Sendromu (CSS) tanısı koyduk. Hastanın montelukast kullanmadan önceki periferik kan incelemelerindeki eozinofil oranları normal iken, ilacı kullanmaya başladıktan sonra eozinofil oranlarının %10’un üzerinde seyrettiği eski kayıtların incelenmesi ile öğrenildi. Bu hastada CSS’nin montelukast kullanımı ile ilişkili olduğu düşünüldü. Sadece montelukast tedavisinin kesilmesi ile hastanın klinik ve radyolojik bulgularında, eozinofil sayısında düzelme (%20) sağlandı. Kortikosteroid veya sitotoksik tedavi vermeksizin iyileşme gösteren montelukast kullanımı ile ilişkili CSS’li bu olguyu sunmayı uygun bulduk.Öğe Susceptibility to Atherosclerosis in Patients with Psoriasis and Psoriatic Arthritis as Determined by Carotid-Femoral (Aortic) Pulse-Wave Velocity Measurement(Ediciones Doyma S A, 2009) Soy, Mehmet; Yildiz, Mustafa; Uyanik, Mehmet Sevki; Karaca, Nuran; Guefer, Goekhan; Piskin, SueleymanIntroduction and objectives. In this study we analyzed the susceptibility to atherosclerosis of patients with psoriasis and psoriatic arthritis (PsA) by determining the femoral-carotid pulse wave velocity (PWV), which is a measure of the viscoelastic properties of blood vessels. Methods. The study included 25 patients with psoriasis (age 18-63 years; 13 male), of whom nine had arthritis, as well as 39 sex- and age-matched healthy control subjects (age 24-70 years, 25 male). The systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate, body mass index (BMI) and waist-to-hip ratio (WHR) of all participants were recorded and, in patients, skin lesions were assessed using the psoriasis area and severity index (PASI). Arterial distensibility was determined by automatic carotid-femoral PWV measurement using the Complior Colson device. Results. Mean PWV, SBP and DBP were significantly higher in psoriatic patients than in control subjects (P=.036, P<.001, and P=.005, respectively). In PsA patients, the mean WHR, SBP, DBP and PWV were all significantly higher than in control subjects (P=.001, P=.031, P=.001, and P=.014, respectively). Conclusions. The carotid-femoral PWV is increased in patients with psoriasis and PsA.Öğe Systemic antineutrophil cytoplasmic antibody vasculitis in a patient with chronic lymphocytic leukemia -: Quite a rare diagnosis(Pergamon-Elsevier Science Ltd, 2007) Pamuk, Guelsuem Emel; Uyanik, Mehmet Sevki; Demir, Muzaffer; Tekguenduez, Emre; Turgut, Burhan; Soy, MehmetThere might be rheumatic manifestations of malignant diseases, especially those of the hematological type. Until now, antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis in chronic lymphocytic leukemia (CLL) has been reported on only very few occasions. Here, we present our patient with Rai stage II CLL who came to us with constitutional symptoms. She turned out to have hematuria with dysmorphic erythrocytes and developed hemoptysis. She had pulmonary-renal syndrome and was diagnosed with p-ANCA positive microscopic polyangiitis. She is currently using prednisolone and cyclophosphamide and undergoing regular hemodialysis. Constitutional symptoms in patients with hematological malignancies should make the physicians consider systemic vasculitis after exclusion of disease-related complications. (c) 2006 Elsevier Ltd.
