An ethics committee approval was obtained for our retrospective study and 90 patients diagnosed with meningioma in our hospital who had preoperative images meeting the appropriate criteria were included. Images were evaluated for conventional finding. Tumor segmentation is performed and then Fractal dimension and lacunarity values are calculated. Among the morphological features, tumor-brain interface, borders, enhancement, necrosis, volume and peritumoral edema findings showed statistically significant difference between patients with typical and atypical meningioma (p<0.05). No other conventional parameters showed any significance. Fractal analysis for outline images of T1 and T2-weighted images and lacunarity values from T1 weighted axial and coronal sequences found to be statistically significant between the two groups (p <0.05). Model 1 (Conventional Findings), Model 2 (Fractal Analysis) and Model 3 (Conventional and Fractal findings together), were created in order to evaluate the diagnostic performance with logistic regression analysis. Model 3 showed the highest diagnostic accuracy rate (88.8%) among the models. In addition to conventional magnetic resonance imaging findings, Fractal analysis values obtained from both T1 and T2-weighed images were found to contribute to the differentiation between typical and atypical meningioma. We think that fractal analysis may become effective marker in the prediction of meningioma grade and contribute to clinical management in the near future.
