In this study, we aimed to assess the extent informed consent is being applied in Trakya University Hospital in accordance to Ministry of Health’s regulations. Methods: A data collecting form has been prepared with respect to Ministry of Health’s relevant regulations and it was applied to 78 inpatients. The form consists of 18 two-point statements and 4 questions towards the patient’s demographic profile. As for descriptive statistics, numbers and percentages have been used for the statements, thus arithmetic mean ± standard deviation (minimum-maximum) has been used for the questions, respectively. Results: Out of all included patients, 47 (60.3 %) participants did not have any knowledge about the Informed Consent Form, 24 (30.8%) did not sign the Form, 48 (61.5%) participants did not read the Form and 49 (62.8%) did not understand the Form. Conclusion: From the results of this study, it can be inferred that the extent patients are being informed is still not on intended levels and is not satisfactory. Informed Consent is still a topic to emphasize, improve and put into practice more effectivelyÖğe A Case Report: Ictal Bradycardia in a Patient With Syncopal Attacks(Trakya Üniversitesi, 2017) Demirci, Koray; Güldiken, BabürhanAims: Ictal bradycardia is characterized as the presence of bradycardia during the ictal discharge and mostly seen in left temporal zone originated seizures. It could lead to conditions such as syncope, asystole and severe injuries. With this case report, it is aimed to emphasize the importance of applying 24-hour video electroencephalographyelectrocardiography monitoring in patients with unexplained loss of consciousnessÖğe Clinical Features and Treatment Strategies of Primary Central Nervous System Lymphoma: a Multicenter Retrospective Study(Trakya Üniversitesi, 2015) Demirci, Koray; Pamuk, Gülsüm Emel; Erkurt, Mehmet Ali; Kılıç, Oğuz; Şıvgın, Serdar; Maden, Muhammet; Kuku, İrfanAims: Primary central nervous system lymphoma is a rare, highly malignant disease with poor prognosis. Thecurrent knowledge about the disease is mostly gathered from prospective clinical trials and the optimal treatment modality is still a matter of debate. In this study it is aimed to acquire more information about the clinical features of the disease and the responses to different treatment modalities. Methods: The archives in Trakya University Hospital, İnönü University Turgut Özal Medical Center, PamukkaleUniversity Hospital and Erciyes University Hospital researched retrospectively. Demographic, treatment and survival data were retrieved and their statistical analysis was performed. As descriptive statistics number and percentages, arithmetic mean ± standard deviation, median (maximum-minimum) were used. Survival analysis was performed using Kaplan- Meier method. Results: Median age of patients at diagnosis was 53 and out of 28 patients 15 (53.6 %) of them were male. As the symptoms of the disease 18 (66.7%) patients presented headache, 15 (53.8%) presented focal deficit. As initial treatment 26 (92.9%) patients received chemotherapy, while 19 (67.9%) patients were treated with surgical resection and the median overall survival time was 7 months. Conclusion: Achieving complete remission as response to the initial treatment is associated with an improvedoverall survival. Other survival analysis to compare the impact of all initial treatment methods on overall survival resulted statistically insignificantÖğe Electrophysiological Correlation and Gradation of Patients Prediagnosed With Carpal Tunnel Syndrome(Trakya Üniversitesi, 2015) Demirci, Koray; Yıldız, Ege Miray; Karahan, Ercan; Kandemir, Yonca; Sönmez, Hilal; Tekataş, AslanAims: In this study, it is aimed to detect electrophysiological correlation of patients who were referred to TrakyaUniversity Medical Research Center Neurology Clinics Electromyography (EMG) lab in 2014 with Carpal Tunnel Syndrome (CTS) pre-diagnosis and grade them based on electrophysiological features. Methods: Electromyography (EMG) records of 174 patients who were sent to electromyography lab in 2014 withCarpal Tunnel Syndrome (CTS) pre diagnosis were scanned retrospectively. Patients were recorded with the data of gender and age. Correlation between pre diagnosis and electrophysiological diagnosis were reported statistically by using Kappa and McNemar tests. Patients were classified based on sex using Chi-square test. Number and percentages, arithmetic mean± standard deviation, median (minimum-maximum) were used as descriptive analysis. Results: Out of all electrophysiological diagnosis, 62 patients (35.6%) were consistent with pre diagnosis when 112(64.4%) were inconsistent. Out of patients who were diagnosed with Carpal Tunnel Syndrome (CTS) according to their electromyography (EMG) findings, 31 patients (48.4%) had mild syndrome. Conclusion: A big inconsistency between pre diagnosis and electromyography (EMG) results was detected. It isimportant to make a more careful and detailed examination to fix this problemÖğe Evaluating the Prevalence of Depression, Sleep Quality and Social Phobia Among Trakya University Hospital Residents(Trakya Üniversitesi, 2016) Berber, Murathan; Gökçe, Kübra; Demirci, Koray; Yiğitbilek, Furkan; Emin, Ahmet; Güldağ, Mahmut Alper; Yumurtacı, ÖznurAims: The purpose of this study is to determine and evaluate the depression, sleep quality and social anxiety levels among medical residents at Trakya University Hospital. Methods: This questionnaire study was conducted among 105 voluntary medical residents at Trakya University Hospital. Beck Depression Inventory, Pitssburgh Sleep Quality Index and Liebowitz Social Anxiety Scale was used to assess depression, sleep quality and social phobia. As for statistical analyses, chi-square, Kruskal Wallis and Mann Whitney Tests were used for analyzing categorical and continuous data. Results: It was found out that 39% of them had depressive symptoms ranging from “mild depressive” to “very severe depressive”, when 14.3% had poor sleep quality and 87.6% had social anxiety symptoms ranging from “moderate social anxiety” to “very severe social anxiety”. Beck Depression Score for surgical residents was significantly higher than basic sciences residents, when female residents had a significantly higher score for social anxiety in comparison with male residents. Conclusion: The prevalence of depression, poor sleep quality and social anxiety among residents was found to be higher than expected which might be due to stress, heavy workload and lifestyle of physiciansÖğe Level of Information About Radiation Among Medical Staff Working in Operating Rooms With Fluoroscopy(Trakya Üniversitesi, 2015) Yıldız, Ege Miray; Kandemir, Yonca; Demirci, Koray; Özcan, MertAims: The aim of this study is to determine the level of information regarding radiation safety and the side effects related to the use of fluoroscopy among a group of teaching assistants, research assistants, medical students, nurses, technicians and caretakers working at Trakya University Health Center for Medical Research and Practice. Methods: A descriptive, cross-sectional survey study was conducted from December 2014 to April 2015 at Trakya University Health Center for Medical Research and Practice. Responses regarding radiation safety and the side effects of fluoroscopy were analyzed in Statistical Package for the Social Sciences (SPSS) 20.0 using descriptive statistics including frequencies and percentages. Results: 98.08% of the participants responded “yes” to the question ”Do you think fluoroscopy is hazardous to health?”. On the other hand, the results showed that only 32.69% of the participants wore lead apron and 32.77% used thyroid shields in every operation. 88.46% of the participants reported that they would like to attend a training regarding fluoroscopy and radiation safety. Conclusion: The results indicated that the operating room staff of Trakya University Health Center for Medical Research and Practice does not possess adequate knowledge regarding radiation safetyÖğe Role of Nitric Oxide in the Antipruritic Effect of WIN 55,212-2, a Cannabinoid Agonist(Iran Univ Medical Sciences, 2020) Gercek, Oyku Zeynep; Oflaz, Busra; Oguz, Neslihan; Demirci, Koray; Gunduz, Ozgur; Ulugol, AhmetIntroduction: For centuries, cannabinoids are known to be effective in pain relief. Itch is an unpleasant sensation that provokes a desire to scratch. Since itch and pain are two sensations sharing a lot in common, we aimed to investigate whether the cannabinoid agonist WIN 55,212-2 reduces serotonin-induced scratching behavior and also observe whether modulation of Nitric Oxide (NO) production mediates the antipruritic effect of WIN 55,212-2. Methods: Scratching behavior is induced by intradermal injection of serotonin (50 mu g/50 mu L/mouse) to BALB/c mice. The cannabinoid agonist WIN 55,212-2 (1, 3, 10 mg/kg, IP) was given 30 min before serotonin injection. To observe the effect of NO modulation on the antipruritic effect of cannabinoids, the endothelial nitric oxide synthase (NOS) inhibitor L-NAME (3 mg/kg, IP), the neuronal NOS inhibitor 7-nitroindazole (3 mg/kg, IP), and the NO precursor L-arginine (100 mg/kg, IP) were administered together withWIN 55,212-2. Results: WIN 55,212-2 reduced serotonin-induced scratches at higher doses (3, 10 mg/kg; P<0.0001). The endothelial NOS inhibitor L-NAME, the neuronal NOS inhibitor 7-nitroindazole, and the nitric oxide precursor L-arginine did not influence the antipruritic action of WIN 55,212-2. When NO modulators were used alone, only the neuronal NOS inhibitor 7-nitroindazole attenuated serotonin-induced scratches (P<0.0001). Conclusion: Our findings indicate that exogenous cannabinoids may attenuate serotonin-induced scratches and NO does not mediate the antipruritic effect of WIN 55,212-2. On the other hand, neuronal NOS inhibition may play a role in the production of serotonin-induced scratches. Highlights Exogenous cannabinoids reduce serotonin-induced scratching behavior. Nitric oxide does not mediate the antipruritic action of cannabinoids. Cannabinoids have the potential to be used as antipruritic drugs. Plain Language Summary Cannabinoid drugs are not used effectively due to their potential drug abuse and side effects. For centuries, cannabinoids are known to exert analgesic actions, but they also produce antipruritic effects. There are numerous studies on the mechanisms of the analgesic effects of cannabinoids; however there are only a few research on their antipruritic mechanism of action. In this study, we observed the modulatory role of nitric oxide in the effect of cannabinoids on serotonininduced scratches which nitric oxide did not play role in this action. We induced scratching behavior by administering serotonin intradermally. Then we injected the cannabinoid agonist WIN 55,212-2 and observed the reduction of the scratching behavior. Afterwards, we administered an endothelial nitric oxide synthase inhibitor, a neuronal nitric oxide synthase inhibitor, and a nitric oxide precursor and showed that nitric oxide does not mediate the antipruritic effects of WIN 55,212-2. Whatever the mechanism of action, cannabinoids have the potential to be used as antipruritic drugs, especially if their side effects are reduced.
