Öğe The Amount of Comorbidities as a Single Parameter Has No Effect in Predicting the Outcome in Appendicitis Patients Older than 60 Years(Lippincott Williams & Wilkins, 2010) Ibis, Cem; Albayrak, Dogan; Hatipoglu, Ahmet R.; Turan, NesrinBackground: Retrospective analysis of acute appendicitis patients older than 60 and statistical evaluation of the association between the accompanying illnesses, severity of peritonitis, morbidity, and mortality rates. Introduction: Although acute appendicitis is not frequent among aged populations, accompanying comorbidities of the geriatric patients are always fear factors before an emergency abdominal surgery even for acute appendicitis. Methods: The data of the 27 patients older than 60 years who underwent appendectomy were retrospectively analyzed. The patients were grouped according to the total number of their comorbidities, Mannheim peritonitis index scores, and the period between the onset of abdominal pain and the admission to the hospital. Results: The mean age of the patients was 73 years. The morbidity and mortality rates were statistically significantly higher in perforated cases. The comparison of patients with <= 1 comorbidity with patients with >= 2 comorbidities did not show any statistical difference in morbidity and mortality. The sensitivity, specificity, positive predictive value, and negative predictive values for mortality in geriatric acute appendicitis patients with Mannheim peritonitis index scores greater than 26 were found to be 75%, 86%, 50%, and 95%, respectively. Discussion: Retrospective evaluation of geriatric patients with acute appendicitis, according to the total number of their comorbidities, surprisingly revealed no statistical difference between groups. We thought that a high Mannheim peritonitis index score is a more reliable criteria than the total number of comorbidities in predicting the prognosis of patients with acute appendicitis in the geriatric population.Öğe Association between specific KRAS mutations and the clinicopathological characteristics of colorectal tumors(Spandidos Publ Ltd, 2015) Kodaz, Hilmi; Hacibekiroglu, Ilhan; Erdogan, Bulent; Turkmen, Esma; Tozkir, Hilmi; Albayrak, Dogan; Uzunoglu, SernazThe aim of this study was to investigate the clinicopathological characteristics and distribution by tumor localization of KRAS point mutations in metastatic colorectal cancer. A total of 189 patients diagnosed with colorectal cancer between 2007 and 2014, who were either metastatic at the time of diagnosis or developed metastasis subsequently, were included in this study. KRAS mutation analysis was performed in the primary tumor tissues and KRAS mutations were identified in 47.6% of the patients. There was a high frequency of the p.G13D point mutation in left-colon tumors (P=0.011), while the p.G12D point mutation was more frequent in right-colon tumors (P=0.004). KRAS wild-type frequency (P=0.02) was higher among patients aged <40 years. A comparison of codon 12 and 13 mutations revealed that codon 12 mutations were more common in the >50-year-old group (P=0.03) and codon 13 mutations were more common in the <70-year-old group (P=0.04). KRAS wild-type tumors were localized in the right colon (P=0.005) and tumors with the p.G13D point mutation (P=0.018) were diagnosed at non-metastatic stages. In conclusion, KRAS point mutations in colorectal cancer exhibited a heterogeneous distribution in terms of tumor localization. In addition, the p.G13D point mutation was found to differ from other mutations in several aspects.Öğe Benign pneumoperitoneum: A case report(Ekin Tibbi Yayincilik Ltd Sti-Ekin Medical Publ, 2008) Albayrak, Dogan; Hatipoglu, Ahmet Rahmi; Ibis, Abdil Cem; Polat, Nihat; Coskun, IrfanFlexible colonoscopy is the gold standard in diagnosis, treatment, and follow-up of colonic pathologies. The acute onset of abdominal pain after colonoscopy may be a clinical sign of colonic perforation. Perforation rate after diagnostic colonoscopies varies between 0.01%-0.4%. If the patient develops signs of peritoneal irritation, the surgical intervention is obligatory. According to the literature, early surgical treatment is the best strategic approach that decreases the morbidity and mortality. Nonoperative treatment could be a choice in selected patients without signs of peritoneal irritation. In large colonoscopy series, only 1-5% of the cases with colonic perforation secondary to the colonoscopy were treated conservatively. Treatment should be started as soon as possible after diagnosis. Material/Methods: Forty 24-week-old male Sprague Dawley rats, with an average weight of 250 g to 300 g, were included in this study. The rats were randomly placed into five groups, each group consisted of eight rats. The sham group underwent only common bile duct (CBD) dissection and no ligation was performed. CBD ligation was applied to the other groups. After the operation, one CBD group was fed with rat chow only, the others were fed with rat chow supplemented with honey, or immunonutrients, or honey plus immunonutrients. After 10 to 12 days, all rats were sacrificed; blood and tissue samples were collected for biochemical, microbiological, and histopathological evaluation. Results: In the groups that were fed with honey and immunonutrients, alanine aminotransferase (ALT) levels were de- creased significantly compared to the other groups. Statistically significant differences were detected in terms of bacterial translocation (BT) rates among liver and spleen samples, and laboratory values of serum, except for MLNs of the BDL+HI group, when compared to other groups. We found mean mucosal thickness of ileum samples have been improved notably in the BDL+HI group compared to the other groups, especially compared to the C/BDL group. Conclusions: Immunonutrition applied with honey had immunostimulant effects, decreased BT due to an additive effect, and had positive effects on intestinal mucosa.Öğe Combined Effects of Tauroursodeoxycholic Acid and Glutamine on Bacterial Translocation in Obstructive Jaundiced Rats(Galenos Publ House, 2013) Hatipoglu, Ahmet Rahmi; Oguz, Serhat; Gurcan, Saban; Yalta, Tulin; Albayrak, Dogan; Erenoglu, Cengiz; Sagiroglu, TamerBackground: Bacterial Translocation is believed to be an important factor on mortality and morbidity in Obstructive Jaundiced. Aims: We investigated the probable or estimated positive effects of tauroursodeoxycholic acid, which has antibacterial and regulatory effects on intestinal flora, together with glutamine on BT in an experimental obstructive jaundiced rat model. Study Design: Animal experimentation. Methods: Forty adult, male, Sprague Dawley rats were used in this study. Animals were randomised and divided into five groups of eight each: sham (Sh); control (common bile duct ligation, CBDL); and supplementation groups administered tauroursodeoxycholic acid (CBDL+T), glutamine (CBDL+G), or tauroursodeoxycholic acid plus glutamine (CBDL+TG). Blood and liver, spleen, MLN, and ileal samples were taken via laparotomy under sterile conditions for investigation of bacterial translocation and intestinal mucosal integrity and hepatic function tests on the tenth postoperative day. Results: There were statistically significant differences in BT rates in all samples except the spleen of the CBDL+TG group compared with the CBDL group (p=0.041, p=0.026, and p=0.041, respectively). Conclusion: It is essential to protect hepatic functions besides maintaining intestinal mucosal integrity in the active struggle against BT occurring in obstructive jaundice. The positive effect on intestinal mucosal integrity can be increased if glutamine is used with tauroursodeoxycholic acid, which also has hepatoprotective and immunomodulatory features.Öğe Comparison between Karydakis flap repair and primary closure for surgical treatment of sacrococcygeal pilonidal sinus(Ekin Tibbi Yayincilik Ltd Sti-Ekin Medical Publ, 2008) Polat, Nihat; Albayrak, Dogan; Ibis, Abdil Cem; Altan, AydinObjectives: In this prospective study, we compared primary closure and Karydakis flap repair which has recently proved a popular technique for surgical treatment of pilonidal sinus. Patients and Methods: The study included 48 patients (46 males, 2 females) who were operated on for pilonidal sinus. The patients were randomized to two groups. After sinus excision, 33 patients (group 1) underwent primary closure, and 15 patients (group 2) underwent Karydakis flap surgery. The mean follow-up was 6.8 months (range 4 to 11 months) in group 1, and 6.2 months (range 3 to 10 months) in group 2. Results: Serous liquid collection and discharge were seen at the site of the lesion in four patients (12.1%) in group 1, and in two patients (13.3%) in group 2. Subcutaneous hematoma was detected in one patient (6.7%) in group 2. During the follow-up period, none of the patients developed wound infection. Recurrences were seen in one patient (3%) in group 1, and in one patient (6.7%) in group 2. Conclusion: Excision and primary closure can be the preferred method for the treatment of pilonidal sinus in selected patients due to its advantages such as ease and simplicity, shorter recovery time, and limited scar formation.Öğe Effect of Dual Mesh Wrap on The Stability of Colonic Anastomosis(Aves, 2021) Albayrak, Dogan; Turkes, Bayram; Can, Nuray; Cakcak, Ibrahim Ethem; Kahya, Eyup; Oguz, Serhat; Hoscoskun, ZekiBACKGROUND/AIMS Colon anastomotic leakage is the cause of significant morbidity and mortality in surgery. Therefore, we aimed to demonstrate experimentally the effect of the conventional method of wrapping them through colo-colonic anastomosis performed by dual patch on anastomotic healing and security. MATERIAL and METHODS The study was conducted at the Experimental Animal Research Unit of the Medical Faculty of Trakya University, Edirne, Turkey. A total of 20 Wistar Albino adult rats were used. The rats were divided into 2 groups: 10 rats in the control group and 10 rats in the dual patch group. All rats underwent partial segmental colon resection, and colo-colonic anastomosis was performed in a single layer with 5/0 polypropylene. For rats in the dual patch group, the anastomosis was wrapped up with a 1-cm-wide dual patch. In the control group, there was no operation on the anastomosis. At 7 days after the procedure, the degree of peritoneal adhesion model previously prepared according to Evans devices laparotomy was used to observe all the rats and colon burst pressure. Both histopathological examination and the examination of the perianastomotic area were performed. RESULTS In the rats that were treated with dual mesh, there was a statistically significant increase in anastomotic line burst pressure (P<.05), a statistically significant increase in collagen amount (P<.05), and a statistically significant decrease in abdominal adhesions (P<.05). CONCLUSION As a result, the colo-colonic anastomosis of winding dual patch, without causing intra-abdominal adhesions, improves security by increasing the anastomosis bursting pressure.Öğe Effect of Short Message Service Use on Bowel Preparation Quality in Patients Undergoing Colonoscopy(Lippincott Williams & Wilkins, 2020) Kizilcik Ozkan, Zeynep; Unver, Seher; Yildiz Findik, Ummu; Albayrak, Dogan; Fidan, SukriyeThis study aimed to determine the effect of SMS use on the bowel preparation quality and satisfaction in patients who underwent colonoscopy. This study was conducted as an experimental-control study with 200 patients who had received colonoscopy in the endoscopy unit of a university hospital in Eastern Thrace of Turkey between December 1, 2016, and June 30, 2017. All patients received a standard bowel preparation. The SMS was sent in addition to the standard bowel preparation to patients in an SMS group. In the SMS group, patients were informed about the standard bowel preparation practices via SMS 3 days before colonoscopy. Patient identification, bowel preparation quality, and patient satisfaction forms were used for data collection. Chi-square and independent sample t tests were used in data analysis. The average age of the participants was 57.57 +/- 12.37 years, and it was determined that most of the participants were male (64.5%) and had colonoscopy experience (55%). The bowel preparation quality was significantly higher in the SMS group than in the no-SMS group (p = .044), and most patients (91%) were very satisfied with being informed via SMS. Sending SMS about bowel preparation practices to colonoscopy patients improves bowel preparation quality and patient satisfaction. Integrating SMS into standard care may help patients manage the bowel preparation process.Öğe Hepatic hydatid cyst cases(Aves Yayincilik, Ibrahim Kara, 2008) Albayrak, Dogan; Sezer, Yavuz Atakan; Ibis, Abdil Cem; Yagci, Mehmet Ali; Hatipoglu, Ahmet Rahmi; Coskun, IrfanObjectives: Hydatid cyst is presently a prevalent health problem in our area and country. This study retrospectively reviewed medical records of four patients who underwent urgent surgery due to intraperitoneal rupture and the records of all patients treated for hepatic hydatidosis in the same period. Patients and Methods: Medical records of the 44 patients (24 females, 20 males; mean age 47.5 years; range 16 to 83 years) who were treated in our clinic for hepatic hydatidosis between January 2004 and June 2007 were reviewed retrospectively. The patients were evaluated with respect to age, sex, location of the cyst, surgical method applied, hospital stay duration, pre- and postoperative complications, and treatment methods applied for the complications. Results: Four patients (9%) underwent emergency surgery due to intraperitoneal rupture. Postoperative biliary leakage into the cyst cavity was observed in 15 patients (34%). It was closed spontaneously in 6 patients (40%) without further intervention. In 8 patients (53%) the biliary leakage continued for about 10 days and the daily median drainage was over 100 cc. Endoscopic sphincterotomy was performed in these cases. Mortality rate was 4% (n=2). Conclusion: Rupture of hydatid cyst should be suspected in trauma patients who had hepatic cyst and intraperitoneal free fluid concomitantly. In these patients therapeutic intervention should be quick because of the risk of any anaflactic reaction. The patients with unresectable metastasis during diagnosis who had undergone resection of primary tumor without chemotherapy and the patients without resection of primary tumor were included. Among patients who had met the inclusion criteria, 46 patients with resection of primary tumor and 47 without resection of primary tumor were included in the study. A total of 93 unresectable metastatic colorectal cancer patients were included in the study. Median PFS was 9 months (95 % CI 7.37-10.62) in patients with resected primary tumor and bevacizumab containing first-line chemotherapy combination. Median PFS was 10 months (95 % CI 8.06-11.93) in patients without bevacizumab (P = 0.66) Median OS was 25 months (95 % CI 17.92-32.07) in patients with resected primary tumor and bevacizumab containing first-line chemotherapy combination. Median OS was 16 months (95 % CI 9.71-22.28) in patients without bevacizumab (P = 0.36) Median OS was 16 months (95 % CI 13.06-8.939) in patients without resected primary tumor and bevacizumab containing first-line chemotherapy combination. Median OS was 9 months (95 % CI 1.48-16.51) in patients without bevacizumab (P = 0.012). Bevacizumab seems ineffective in mCRC patients with resected primary tumor. An increase in number of retrospective literature data and randomized, prospective studies is required about this subject.Öğe Impact of Gentamicin-Impregnated Collagen on the Intra-Abdominal Adhesions and Integrity of Colonic Anastomosis: An Experimental Study(Int Scientific Information, Inc, 2021) Oruc, Esin Erturer; Albayrak, DoganBackground: We investigated the impact of collagen usage in colo-colonic anastomoses on intra-abdominal adhesion and anastomosis safety. Material/Methods: A total of 30 adult albino Wistar rats (aged 6-8 months) weighing 180-230 g in the laboratory setting were used in this study. Rats were divided into the 3 groups, consisting of 10 rats in each group: treated with gentamicin-impregnated collagen, treated with only collagen, and the control group. After 7 days, rats were sacrificed to evaluate adhesion scores and anastomosis bursting pressures. The Evans scoring system was used to rate adhesion levels. Bursting pressures were measured using a handheld tension device, and the scores obtained at the moment of tissue dissection were determined as the bursting pressure. Results: The mean adhesion scores were 2.86 +/- 0.37 in the control group, 1.80 +/- 0.91 in the collagen group, and 1.78 +/- 0.83 in the gentamicin-impregnated collagen group, with the control group showing significantly higher scores than the other groups (p=0.010 and p=0.011, respectively). The mean bursting pressure levels were 174.29 +/- 44.68 mmHg in the control group, 223 +/- 38.6 mmHg in collagen group, and 223.33 +/- 42 mmHg in the gentamicin-impregnated collagen group, showing that the mean bursting pressure levels were significantly lower in the control group than the other groups (p=0.027 and p=0.029, respectively). Conclusions: This study suggests that colo-colonic anastomosis coverage using materials incorporating collagen alone or gentamicin-impregnated collagen increases the safety of anastomosis and reduces intra-abdominal adhesions.Öğe Intussusception due to fibroid polyp: case report(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2008) Ibis, Abdil Cem; Albayrak, Dogan; Sezer, Yavuz Atakan; Polat, Nihat; Coskun, IrfanIntusussusception is a clinical aspect characterized by the entrance of proximal intestinal segments into distal part. Ileal intussusception is a disease for child age group. Only 5% of all intussusception is seen in adults. Contrary to child age group in many of the adult patients, there is a lesion causing intussusception. Therefore, a surgical cure is necessary for most of the patients. Although the type of surgical treatment is still controversial, partial resection rather than reduction and primary anastomosis are commonly recommended especially for adults. In this article, an adult patient diagnosed as ileo-ileal intussusception during surgery for mechanical intestinal obstruction and treated by small bowel resection and primary anastomosis is presented.Öğe Naringin Combined with NF-?B Inhibition and Endoplasmic Reticulum Stress Induces Apoptotic Cell Death via Oxidative Stress and the PERK/eIF2?/ATF4/CHOP Axis in HT29 Colon Cancer Cells(Springer/Plenum Publishers, 2021) Albayrak, Dogan; Doganlar, Oguzhan; Erdogan, Suat; Merakli, Meryem; Dogan, Ayten; Turker, Pelin; Bostanci, AytenCurrently, combination therapy is considered the most effective solution for a selective chemotherapeutic effect in the treatment of colon cancer. This study investigated the death of both colon cancer HT29 cells and healthy vascular smooth muscle TG-Ha-VSMC cells (VSMCs) induced by naringin combined with endoplasmic reticulum (ER) stress and NF-kappa B inhibition. Naringin combined with tunicamycin and BAY 11-7082 suppressed the proliferation of HT29 cells in a dose-dependent manner and induced particularly apoptotic death without significantly affecting healthy VSMCs according to Annexin V/PI staining and AO/EB staining analyses. Insufficient antioxidant defense and heat shock response as well as excessive ROS generation were observed in HT29 cells following combination therapy. Quantitative real-time PCR and western blot analysis demonstrated that drug combination-induced mitochondrial apoptosis was activated through the ROS-mediated PERK/eIF2 alpha/ATF4/CHOP pathway. Additionally, naringin combination significantly reduced the sXBP expression induced by tunicamycin+BAY 11-7082 in a dose-dependent manner. In conclusion, this study found that naringin combined with tunicamycin+BAY 11-7082 efficiently induced apoptotic cell death in HT29 colon cancer cells via oxidative stress and the PERK/eIF2 alpha/ATF4/CHOP pathway, suggesting that naringin combined with tunicamycin plus BAY 11-7082 could be a new combination therapy strategy for effective colon cancer treatment with minimal side effects on healthy cells.Öğe Perforated primary small bowel lymphoma: A case report(Ekin Tibbi Yayincilik Ltd Sti-Ekin Medical Publ, 2008) Albayrak, Dogan; Ibis, Abdil Cem; Hatipoglu, Ahmet Rahmi; Polat, Nihat; Hoscoskun, ZekiMalignant tumors of the small bowel are rarely seen and are usually diagnosed in advanced stages because of the difficulties in diagnosis. Lymphomas account for %1 of malignant gastrointestinal system tumors. Primary gastrointestinal lymphoma is very rare and its preoperative diagnosis is difficult. Its diagnosis is usually made through laparotomy. A 77-year-old male patient developed acute abdomen during observation for partial mechanic intestinal obstruction. Emergent explorative laparotomy revealed perforation of a small bowel tumor. A wide small bowel resection was performed with a double-layer end-to-end anastomosis. Histopathological examination of the surgical specimen was reported as indolent B-cell non-Hodgkin's lymphoma. It should be kept in mind that small bowel tumors can present as mechanic intestinal obstruction, resulting in delayed diagnosis.Öğe Primary Hydatid Disease of Pancreas Mimicldng Cystic Neoplasm(Lippincott Williams & Wilkins, 2009) Ibis, Cem; Albayrak, Dogan; Altan, AydinPrimary hydatid disease of the pancreas is very rare. We report a 33-year-old female who was admitted to the hospital with abdominal discomfort due to the pancreatic mass. A diagnosis of a pancreatic cystic mass was established through abdominal ultrasonography and computed tomography scan. Hydatid disease as well as a cystic neoplasm of the pancreas were both thought in the differential diagnosis. Distal pancreatectomy with splenectomy was performed. The histopathologic evaluation of the specimen revealed a hydatid cyst affecting the tail of the pancreas. Hydatid disease Should be considered in the differential diagnosis of all cystic masses of the pancreas, especially in endemic regions.Öğe Primary Hydatid Disease of Retroperitoneum: Case Report(Derman Medical Publ, 2013) Albayrak, Dogan; Ibis, Abdil CemHydatid disease is still seen as an endemic disease in several regions of the world and holds importance for both our country and region as a common health problem. In this study, a chronic atrial fibrilation patient taking anticoagulant, had perforation at rectosigmoid region which was secondary to intramural hematoma, is presented. Our case was a eighty-three-year-old male presented with acute abdomen at emergency room. In computerized tomography, intraabdominal free air and hematoma at rectosigmoid region were seen. Due to these, the decision of immidiate exploration was made. This should be noted that intramural hematoma can emerge in patientd on anticoagulant theraphy without trauma. Additionally, it shold be considered that intramural hematoma can lead to lethal complication such as perforation.Öğe Rectal foreign bodies: A report of three cases(Ekin Tibbi Yayincilik Ltd Sti-Ekin Medical Publ, 2007) Albayrak, Dogan; Ibis, Abdil Cem; Hatipoglu, Ahmet Rahmi; Irfanoglu, Mehinet EminDifferent foreign bodies were found in rectum in three male patients, whose ages were 25, 66 and 67. These foreign materials were iron stick, special wooden stick and tea cup. In two patients, materials were taken out under general anesthesia in lithotomy position and the other one in knee-elbow position by anal dilatation under local anesthesia with sedation. In two patients there were not any complications related with the treatment method but one of them had superficial lacerations on rectal mucosa. All of the patients were discharged after 24 hours observation. Foreign bodies in rectum and sigmoid colon is increasingly being seen recently and is one of the significant surgical emergency problems due to the complications it may cause.
